磁共振扩散加权成像在不同类型正常乳腺组织中的应用 被引量:2

Application of diffusion-weighted magnetic resonance imaging in different types of normal breast tissues

摘要 目的探讨不同类型正常乳腺组织与其表现扩散系数值(ADC)是否存在相关性。方法选取2008年1月至2013年12月在我院同时行乳腺X线摄影及乳腺MRI检查的78例女性患者,根据美国放射学会制定的乳腺影像报告和数据系统将乳腺分为四型:致密型(9例),多腺体型(27例),少腺体型(26例),脂肪型(16例)。乳腺X线摄片体位为头尾位及内侧斜位片,MRI检查行平扫、动态增强扫描及MR扩散加权成像(DWI)序列,扩散敏感因子b=800/mm2。分别测量各组乳腺的ADC值并做统计。结果各组ADC均值中致密型(1.5±0.000 6)×10-3mm2/s,多腺体型(1.7±0.000 5)×10-3mm2/s,少腺体型(1.7±0.000 4)×10-3mm2/s,脂肪型(1.1±0.000 5)×10-3mm2/s,经单因素方差分析,其差异具有统计学意义(F=6.317,P<0.01);其中致密型与多腺体型间,致密型与少腺体型间,致密型与脂肪型间,以及多腺体与少腺体型间分别两两比较差异均无统计学意义(P>0.05),而多腺体与脂肪型间和少腺体型与脂肪型间两两比较,差异均有统计学意义(P<0.01)。结论不同类型正常乳腺与其组织的ADC值有一定的相关性。 Objective To explore whether values of apparent diffusion coefficient(ADC) were related to different types of normal breast tissue. Methods Seventy-eight patients who underwent breast X-rayphotography and breast magnetic resonance imaging(MRI) from January 2008 to December 2013 were selected. Based on Breast Imaging Reporting and Data System developed by American College of Radiology, the breasts were divided into four types: dense type(9 cases), multiple gland type(27 cases), less gland type(26 cases), fat type(16 cases). All patients took the heads and inside of the diagonal slice in breast X-ray radiography, as well as MRI scanning and dynamic enhanced scanning and MR diffusion-weighted imaging(DWI) examination, with diffusion sensitive factor b of 800/mm2.The value of ADC was measured and statistically analyzed respectively in each breast group. Results The mean value of ADC in each group was as follows: dense type(1.5±0.000 6)×10-3mm2/s, multiple gland type(1.7±0.000 5)×10-3mm2/s,less gland type(1.7±0.000 4)×10-3mm2/s, fat type(1.1±0.000 5)×10-3mm2/s. The differences were statistically significant by one-way ANOVA(F=6.317, P〈0.01). There were no statistically significant differences when compared dense type to multiple gland type, dense type to less gland type, dense type to fat type, as well as multiple gland type to less gland type(P〉0.05). There were statistically significant differences between multiple gland type and fat type, less gland type and lard type(P〈0.01). Conclusion The values of ADC are correlated with different types of normal breast tissue.
