摘要
目的分析不同类型正常乳腺不同月经周期表观扩散系数的差异,为乳腺疾病的诊断提供依据。方法 55例20~49岁月经正常妇女,于晚卵泡期、黄体晚期行磁共振平扫及扩散加权成像(DWI)检查未发现乳腺疾患,根据Wolf分型,致密型16例,分叶串珠型39例,扩散敏感因子b取0、1000s/mm2,测量双乳乳头层面腺体平均ADC值。结果双乳晚卵泡期ADC值明显低于黄体晚期(t=-5.126、-5.896,P<0.01);晚卵泡期致密型腺体ADC值明显高于分叶串珠型腺体(t=2.186、2.495,P<0.05),黄体晚期致密型腺体ADC值与分叶串珠型差异无统计学意义(t=2.003、1.856,P>0.05)。结论乳腺腺体ADC值晚卵泡期降低明显,致密型腺体ADC值受月经周期影响较分叶串珠型腺体小。
Objective To analyse the differences of apparent diffusion coefficient( ADC) in different kinds of normal breast glands during different menstrual cycles,and to provide the diagnostic proof of breast diseases. Methods Fifty-five cases of women aged 20 ~ 49,had normal menstruation were examined in later follicular phase and later corpus luteum phase,respectively by MR plain scan and MR diffusion weighed imaging( DWI) ,and no breast disease was found. Wolf typing showed dense form in 16 cases and sublobe-hiaqua form in 39 cases. The average mammary gland ADC values were measured at the bilateral nipple level when b value was given from 1000 to 0 s/mm2. Results The ADC value of bilateral mammary gland in later follicular phase was markedly lower than that in the later corpus luteum phase( P 0. 01) ,in which ADC value of the dense form mammary gland was obviously higher than that of the sublobe-hiaqua form mammary gland( P 0. 05) . The difference between the ADC values of dense form mammary gland and the sublobe-hiaqua form mammary gland in later corpus luteum phase had no statistical significance. Conclusion The ADC values of mammary gland decreases significantly in later follicular phase. Comparing with the sublobe-hiaqua form mammary gland,the ADC value of dense form mammary gland is less influenced by the menstruation cycle.
