Angiogenesis of hepatocellular carcinoma and its clinicopathological significance

摘要 目的 探讨肝细胞肝癌 (HCC)肿瘤血管生成与临床病理的意义 ,及其调控因素 p5 3与血管内皮生长因子 (VEGF)和微血管密度 (MVD)之间的内在联系。方法 经手术治疗的HCC 6 0例 ,分别进行HE染色和 p5 3、VEGF和CD34的免疫组化染色。根据CD34阳性结果计数测定MVD。记录HCC患者的手术及临床病理特征如AFP结果、包膜情况、肝内转移、病理分级汇管区癌栓等资料 ,并将 p5 3、VEGF和MVD结果分别与上述指标进行统计分析。 结果 MVD值在汇管区癌栓组 (2 2 0 76± 89 84 )和肝内转移组 (2 2 5 31± 87 17)高于无汇管区癌栓组 (16 8 79± 87 5 7)和无肝内转移组 (173 0 5± 89 4 0 ) ,差异有显著性 (P <0 0 5 )。VEGF表达的阳性率在汇管区癌栓组 (71 4 3% )、肝内转移组 (75 % )和包膜完整组 (75 % )中均高于无汇管区癌栓组 (4 1 0 3% )、无肝内转移组 (4 3 18% )和无包膜 (4 0 % )或包膜不完整组 (35 2 9% ) ,差异有显著性 (P <0 0 5 )。p5 3蛋白表达的阳性率在汇管区癌栓组 (5 7 14 % )、肝内转移组 (6 2 5 0 % )和中低分化组 (4 1 30 % )中均高于无汇管区癌栓组(2 0 5 1% )、无肝内转移组 (2 2 75 % )和高分化组 (7 14 % ) ,差异有显著性 (P <0 0 5 )。当 p5 3蛋白表达阳性时 ,MVD值增高(2 2 1 90± Purpose To investigate the relationship between the angiogenesis of hepatocellular carcinoma (HCC) and its clinicopathologic features, and analyze the associations among microvessel density (MVD), p53 and vascular endothelial growth factor (VEGF). Methods Sections of specimen in 60 HCC cases that underwent surgical treatment were stained immunohistochemically with p53, VEGF and CD34. MVD was highlighted by anti CD34. Clinicopathological characteristics of AFP, the tumor capsule, portal vein tumor thrombosis (PVTT), intrahepatic metastasis and histological grades were recorded in surgery and pathology. The MVD, p53 and VEGF were compared with the clinicopathological characteristics respectively. Results The MVD of HCC in the groups with PVTT (220 76±89 84) and intrahepatic metastasis (225 31±87 17) were greater than that in those without PVTT (168 79±87 57) or nonmetastasis (173 05±89 40) ( P <0 05). VEGF expression was significantly different between intrahepatic metastasis (75%) and nonmetastasis (43 18%) ( P <0 05), vascular invasion (71 43%) and noninvasion (41 03%) ( P <0 05). p53 expression showed a distinctly higher among these groups such as poorly and moderately differentiation (41 30%) to well differentiation (7 14%) ( P <0 05), vascular invasion (57 14%) to noninvasion (20 51%) ( P <0 05), intrahepatic metastasis (62 5%) to nonmetastasis (22 75%) ( P <0 05). Significant correlation was also found among p53, VEGF and MVD. Conclusions There is a statistically significant correlation between VEGF and p53 expression ( P <0 05). VEGF associated with p53 influences MVD. Both p53 and VEGF have a significantly improved efficacy in promoting tumor angiogenesis and metastasis in HCC.
