摘要
目的探讨腹部双蒂真皮下血管网皮瓣治疗手指远节软组织挫灭伤合并指骨骨折的临床效果。方法选取2018年1月至2020年12月我院收治的60例手指远节软组织挫灭伤合并指骨骨折患者作为研究对象,按照随机数字表法将其分为对照组和观察组,各30例。对照组采用传统腹部带蒂皮瓣治疗,观察组采用腹部双蒂真皮下血管网皮瓣治疗。比较两组的治疗效果。结果观察组的治疗优良率为96.67%,显著高于对照组的80.00%(P<0.05)。治疗后,观察组的创面组织病理学评分、手指远节屈伸活动度、手指功能评分、世界卫生组织生存质量测定量表简表(WHOQOL-BREF)各维度评分显著高于对照组,创面疼痛、创面瘙痒评分均显著低于对照组(P<0.05)。结论腹部双蒂真皮下血管网皮瓣治疗手指远节软组织挫灭伤合并指骨骨折效果显著,有利于改善患者手指远节屈伸活动度和手指功能,提高其生活质量。
Objective To investigate the clinical effect of abdominal double pedicle subdermal vascular network flap in the treatment of finger distal soft tissue contusion complicated with phalangeal fracture.Methods A total of 60 patients with finger distalsoft tissue contusion complicated with phalangeal fracture admitted in our hospital from January 2018 to December 2020 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,with 30 cases in each group.The control group was treated with traditional abdominal pedicle flap,and the observation group was treated with abdominal double pedicle subdermal vascular network flap.The therapeutic effects of the two groups were compared.Results The excellent and good rate of treatment in the observation group was 96.67%,which was significantly higher than 80.00%in the control group(P<0.05).After treatment,the wound histopathological score,finger distal flexion and extension activity,finger function score,and each dimension scores of World Health Organization Quality of Life Brief Scale(WHOQOL-BREF)in the observation group were significantly higher than those in the control group,and the wound pain and wound itching scores were significantly lower than those in the control group(P<0.05).Conclusion Abdominal double pedicle subdermal vascular network flap in the treatment of finger distal soft tissue contusion complicated with phalangeal fracture has a significant effect,it is beneficial to ameliorate the finger distal flexion and extension activity and the finger function of patients,and improve their quality of life.
