摘要
目的 探讨电脑中频治疗仪联合电子针疗仪在腰椎间盘突出致坐骨神经痛中的应用效果及对血清炎症因子的影响。方法 90例腰椎间盘突出致坐骨神经痛患者,按随机数字表法分为对照组和观察组,每组45例。对照组单纯给予电子针疗仪治疗,观察组给予电脑中频治疗仪联合电子针疗仪治疗。比较两组患者治疗前及治疗8周结束时视觉模拟评分法(VAS)、现时疼痛强度(PPI)、Oswestry功能障碍指数量表(ODI)、改良罗兰-莫里斯残疾问卷(RMDQ)、世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分及炎症因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平,临床疗效,不良反应发生情况。结果 治疗8周结束时,两组患者VAS、PPI、ODI、RMDQ评分低于本组治疗前,WHOQOL-BREF评分高于本组治疗前,且观察组患者VAS、PPI、ODI、RMDQ评分分别为(2.20±0.90)、(2.02±0.80)、(10.98±2.52)、(2.28±0.45)分,低于对照组的(4.25±1.18)、(2.65±0.96)、(17.45±2.32)、(5.84±0.66)分,WHOQOL-BREF评分(72.25±2.33)分高于对照组的(66.26±2.65)分,差异具有统计学意义(P<0.05)。治疗8周结束时,两组患者hs-CRP、IL-6、TNF-α水平均低于本组治疗前,且观察组患者hs-CRP、IL-6、TNF-α水平分别为(9.02±2.25)mg/L、(51.08±6.25)pg/ml、(22.75±3.46)pg/ml,低于对照组的(10.45±2.40)mg/L、(94.75±8.46)pg/ml、(55.18±6.60)pg/ml,差异具有统计学意义(P<0.05)。观察组患者临床总有效率为95.56%(43/45),明显高于对照组的80.00%(36/45),差异具有统计学意义(P<0.05)。两组患者治疗过程中未见明显不良反应。结论 电脑中频治疗仪联合电子针疗仪干预能有效改善腰椎间盘突出致坐骨神经痛患者的疼痛症状,促进腰椎功能恢复,改善躯体活动受限及生活质量,减轻局部炎症反应,值得临床推广应用。
Objective To discuss the application effect of computerized intermediate frequency therapy apparatus combined with electronic needle therapy device in lumbar disc herniation causing sciatica and its influence on serum inflammatory factors.Methods A total of 90 patients with lumbar disc herniation causing sciatica were divided into control group and observation group according to the random numerical table,with 45 cases in each group.The control group was simply treated with electronic needle therapy device,and the observation group was treated with computerized intermediate frequency therapy apparatus and electronic needle therapy device.Both groups were compared in terms of Visual Analogue Scale(VAS)score,Present Pain Intensity(PPI)score,Oswestry Disability Index(ODI)score,Roland-Morris Disability Questionnaire(RMDQ)score,WHO Quality of Life Scale Abbreviated Version(WHOQOL-BREF)score and inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]levels before and at the end of 8 weeks of treatment,clinical efficacy,and occurrence of adverse reactions.Results At the end of 8 weeks of treatment,VAS,PPI,ODI and RMDQ scores in the two groups were lower than those before treatment in this group,while WHOQOL-BREF score was higher than that before treatment in this group;VAS,PPI,ODI and RMDQ scores were(2.20±0.90),(2.02±0.80),(10.98±2.52)and(2.28±0.45)points in the observation group,which were lower than those of(4.25±1.18),(2.65±0.96),(17.45±2.32)and(5.84±0.66)points in the control group;WHOQOL-BREF score of(72.25±2.33)points in the observation group was higher than that of(66.26±2.65)points in the control group;the differences were statistically significant(P<0.05).At the end of 8 weeks of treatment,the levels of hs-CRP,IL-6 and TNF-αin the two groups were lower than those before treatment in this group;the levels of hs-CRP,IL-6 and TNF-αwere(9.02±2.25)mg/L,(51.08±6.25)pg/ml and (22.75±3.46) pg/ml in the observation group, which were lower than those of (10.45±2.40) mg/L, (94.75± 8.46) pg/ml and (55.18±6.60) pg/ml in the control group;the differences were statistically significant (P<0.05). The total clinical effective rate of the observation group was 95.56%(43/45), which was significantly higher than that of 80.00% (36/45) of the control group, and the difference was statistically significant (P<0.05). No obvious adverse reactions were observed in the two groups during treatment. Conclusion Computerized intermediate frequency therapy apparatus combined with electronic needle therapy device can effectively improve the pain symptoms of patients with lumbar disc herniation causing sciatica, promote the recovery of lumbar spine function, improve the limitation of somatic activities and quality of life, and reduce the local inflammatory reaction, which is worthy of clinical promotion and application.
