皮支链皮瓣在手指创伤软组织缺损修复中的应用效果 被引量:3

摘要 背景:目前临床上应用的常规皮瓣均对手指损伤有一定帮助,但同时存在不同程度的弊端,皮支链皮瓣独特的生理解剖结构可在一定程度上促进缺损处感觉功能的恢复,但目前在手指创伤中的应用较少。目的:皮支链皮瓣在手指创伤软组织缺损修复中的应用价值。方法:前瞻性纳入2016年1月至2018年6月收治的手指软组织缺损患者56例,按照随机数字法随机分为研究组28例,采用手部皮支链皮瓣治疗;对照组28例,采用腹部带蒂皮瓣修复治疗。比较两组的手术时间、住院时间、总费用,观察术后12个月的手功能恢复情况及皮瓣情况。结果:研究组的平均手术时间长于对照组,但差异无统计学意义(P>0.05);研究组平均住院时间短于对照组,差异有统计学意义(P<0.05);研究组平均总费用低于对照组,差异有统计学意义(P<0.05)。术后12个月的手功能恢复情况及皮瓣情况研究组均优于对照组,差异有统计学意义(P<0.05)。结论:手部皮支链皮瓣治疗手指软组织缺损术后12个月的手功能恢复及皮瓣恢复较好,且住院时间较短,总费用较低,值得推广应用。 Background:At present,the conventional skin flaps used in clinic are helpful to finger injury,but there are also some drawbacks.The unique physiological and anatomical structure of the branch chain skin flap can promote the recovery of sensory function at the defect to a certain extent,but it is seldom used in finger injury at present.Objective:To explore the application value of skin branch chain flap in repairing soft tissue defect of finger trauma.Methods:Fifty-six patients with finger soft tissue defect treated in our hospital from January 2016 to June 2018 were enrolled in the study.According to random number method,the patients were randomly divided into study group(n=28,hand skin branch chain flap treatment)and control group(n=28,abdominal pedicled skin flap treatment).The operation time,hospitalization time,total cost,hand function recovery and skin flaps 12 months after operation in the two groups were compared and analyzed.Results:The average operation time of the study group was longer than that of the control group,but there was no significant difference(P>0.05);the average hospitalization time of the study group was shorter than that of the control group(P<0.05);the average total hospitalization cost of the study group was lower than that of the control group(P<0.05).The recovery of hand function and the condition of skin flaps in the study group were better than those in the control group 12 months after operation(P<0.05).Conclusions:Hand branch chain skin flaps in the treatment of finger soft tissue defect is worth being promoted for relatively short hospitalization time,low total cost,good hand function and skin flaps recovery 12 months after operation.
