摘要
目的探讨超早期与早期微创手术治疗高血压性脑出血(HICH)患者的临床效果。方法选取2015年1月至2020年12月泉州医学高等专科学校附属人民医院收治的335例中等量基底节区HICH患者为研究对象,将从发病到手术时间≤6 h的70例患者作为试验组,将从发病到手术时间>6 h、<24 h的265例患者作为对照组。两组均实施微创手术治疗,比较两组意识障碍程度[采用格拉斯哥昏迷指数(GCS)评分]、术后1个月的并发症发生率和血肿清除率、术后半年的日常生活活动能力[采用日常生活活动能力(ADL)评估]及病死率。结果入院时,两组GCS评分比较,差异无统计学意义(P>0.05);术后7 d,两组GCS评分均高于入院时,且试验组高于对照组,差异有统计学意义(P<0.05)。试验组术后1个月并发症总发生率低于对照组,血肿清除率高于对照组,差异有统计学意义(P<0.05)。试验组术后半年ADL分级优于对照组,病死率低于对照组,差异有统计学意义(P<0.05)。结论与早期手术相比,超早期手术治疗中等量基底节区HICH患者的效果更佳,对改善患者意识障碍,降低术后并发症发生率,提高血肿清除率及日常生活活动能力的效果更为显著。
Objective To compare the clinical effects of ultra early and early minimally invasive surgery in the treatment of hypertensive intracerebral hemorrhage.Methods 335 patients with moderate basal ganglia hypertensive intracerebral hemorrhage treated in our hospital from January 2015 to December 2020 were selected as the research objects;The patients were divided into groups according to the time from onset to operation.70 patients with onset time≤6h were regarded as the super early group,and 265 patients with onset time>6h and<24h were regarded as the early group;The GCS score,hematoma clearance rate one month after operation,complication rate and ADL grade half a year after operation were compared between the two groups.Results The GCS score of the super early group was significantly higher than that of the early group(P<0.05);Comparing the hematoma clearance rate of the two groups one month after operation,it can be seen that the ultra early group is significantly higher than the early group,and the incidence of complications in the ultra early group is significantly lower than that in the early group(P<0.05);Comparing the ADL grades of the two groups half a year after operation,the super early group was significantly better than the early group(P<0.05).Conclusions In the treatment of moderate basal ganglia hypertension,ultra early treatment is more effective and safe than early treatment.It is recommended to be popularized.
