显微镜下经侧裂岛叶与颞叶皮质入路早期手术治疗基底节区高血压脑出血的效果对比 被引量:5

摘要 目的比较显微镜下经侧裂岛叶与颞叶皮质入路早期手术治疗基底节区高血压脑出血的效果。方法选取2016年1月至2018年1月我院收治的100例基底节区高血压脑出血患者,随机将其分为研究组和对照组各50例。研究组行显微镜下经侧裂岛叶入路早期手术,对照组行颞叶皮质入路早期手术,比较两组的治疗效果。结果研究组的手术时间和术后清醒时间均短于对照组,术中出血量少于对照组(P均<0.05)。术后3个月,研究组的生活自理能力评分高于对照组(P<0.05)。研究组的血肿清除率高于对照组(P<0.05)。两组的术后3 d再出血率及并发症发生率比较无统计学差异(P>0.05)。结论显微镜下经侧裂岛叶入路早期手术治疗基底节区高血压脑出血效果显著、安全性高,患者的生活自理能力强,值得临床推广应用。 Objective To compare the effects of early surgical treatment of hypertensive intracerebral hemorrhage in basal ganglia via transsylvian transinsular approach and temporal cortex approach under microscope.Methods 100 cases of patients with hypertensive intracerebral hemorrhage in basal ganglia admitted to our hospital from January 2016 to January 2018 were selected and randomly divided into study group and control group,with 50 cases in each group.The study group received early surgery through the transsylvian transinsular approach under microscope,and the control group received early surgery through the temporal cortex approach.The treatment effects of the two groups were compared.Results The operation time and postoperative awake time of the study group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group(all P<0.05).3 months after operation,the score of self-care ability of the study group was higher than that of the control group(P<0.05).The hematoma clearance rate of the study group was higher than that of the control group(P<0.05).No statistical difference was found in the rebleeding rate 3 d after operation and the incidence of complications between the two groups(P>0.05).Conclusions Early surgical treatment of hypertensive intracerebral hemorrhage in basal ganglia through transsylvian transinsular approach under microscope has significant effect and high safety,with high self-care ability in patients,which is worthy of clinical promotion and application.
