国医大师尼玛应用藏医药治疗“真布”病的用药规律研究 被引量:5

A Research on the Application of Tibetan Medicine by National TCM Master Nima for the Treatment of “Zhen Bu” Disease

摘要 目的研究国医大师尼玛藏治疗"真布"病的用药规律。方法收集和整理国医大师尼玛处方数据库中治疗"真布"病的方剂。运用IBM SPSS Statistics 24.0和SPSS Modeler 18.0软件进行藏药频数、关联择时用药、聚类分析、决策树分析。结果共纳入82首方剂,频次≥37的有5首方剂,以清热止痛、干黄水、舒筋活络、消肿散结类方剂为主;频次≥8的高频藏药34味,以活血、消肿、止痛类药物为主。方剂药性中以苦、甘、辛等药味为主,三化味中以苦化味为主;药味的药性值中凉、钝、重为最高,分别对应治疗赤巴的热,锐和隆的轻等特性。择时用药规律中十三味鹏鸟丸、二十五味儿茶丸、十八味党参丸、三十五味沉香丸是4个时段常用组合方剂。方剂关联组合均以二十五味儿茶丸、十三味鹏鸟丸、十八味党参丸等三种方剂为轴心进行加减方剂配伍;药物关联规则以清热凉血、干黄水、调和气血的药物为主。药物聚类分析较有别于关联规则的结果,反应了生胃火、解毒的用药规律。因子分析共得到8个公因子,反应了清热消肿、祛瘟解毒的治法特点。结论国医大师尼玛治疗"真布"病以干黄水类药为主、清热止痛类药为辅、并注重隆的调节为治疗原则。 Objective To study the prescription regularity of the treatment of "Zhen Bu" disease by National TCM Master Nima.Methods Prescriptions were collected and compiled from the National TCM Nimas prescription database for the treatment of"Zhen Bu"disease. The IBM SPSS Statistics 24.0 and SPSS Modeler 18.0 software were used to perform the frequency of Tibetan medicine, association of prescription selection, clustering analysis, and decision tree analysis. Results A total of 82 formulas were included, with 5 formulas with a frequency ≥ 37, mainly in the categories of clearing heat and relieving pain, drying yellow water, soothing tendons and activating the muscles, and reducing swelling and dispersing knots;34 high-frequency Tibetan medicines with a frequency ≥ 8, mainly in the categories of activating blood, reducing swelling and relieving pain. The medicinal properties of the formula were dominated by bitter,sweet and pungent flavors, and the three postdigestive flavors were dominated by bitter;the medicinal values of the flavors are highest in cool, blunt and heavy, which correspond to the characteristics of heat for treating Chiba, and light for Ruihelong, respectively. The drug selection rules of Thirteen Flavor Penghu Pill, Twenty-five Flavor Ercha Pill,Eighteen Flavor Party Ginseng Pill, and Thirty-five Flavor Shenxiang Pill are the 4 time periods commonly used combinations of prescriptions. The prescription association combinations were all based on the three prescriptions of Twenty-five Flavor Ercha Pills, Thirteen Flavor d Penghu Wan, and Eighteen Flavor Party Ginseng Pills as the axes for the addition and subtraction of prescription combinations;the drug association rules were based on drugs that clear heat and cool blood, dry yellow water, and harmonize qi and blood. The drug clustering analysis was more different from the results of association rules, which responded to the dosing rules of generating stomach fire and detoxification. Factor analysis yielded a total of 8 common factors, which responded to the therapeutic characteristics of clearing heat and swelling, dispelling plague and detoxifying. Conclusion National TCM Nima treats"Zhen Bu"disease with medicine of drying yellow water, supplemented by medicine of clearing heat and relieving pain, and focusing on the regulation of Long as the treatment principle.
