宫颈高级别病变HPV感染型别分析 被引量:5

摘要 目的探讨人乳头瘤病毒(human papilloma virus,HPV)在宫颈高级别病变中的感染型别及感染率。方法统计2017年1月至2018年6月在扬州市妇幼保健院妇科就诊,并已被确诊为宫颈高级别病变的287人HPV型别感染情况,分别进行单一型别HPV感染率比较;不同类型HPV型别感染率比较;不同年龄段HPV型别感染率比较;不同类型疫苗所包含HPV型别病人感染比率比较,分析宫颈病变的可能性。结果在287人中,有257人HPV阳性。HPV感染单一型别比较以HPV16亚型感染率最高,131例(50.97%);不同类型HPV型别感染中以单一型别感染率最高,159例(66.87%);不同年龄段HPV感染中以30-50岁感染率最高,165例(64.21%);不同类型疫苗中以九价疫苗所包含的型别病人感染率最高,237例(91.51%)。结论 HPV型别感染对高级别病变及宫颈癌病变的影响较大,HPV感染检查对宫颈癌筛查具有诊断意义。四价疫苗已不太能满足临床需要,有条件的女性应尽早进行九价疫苗的接种,另发现在九价疫苗包含的亚型以外,其他型别感染率也较高,建议在新的疫苗中是否可适当增加新的型别。 Objective To investigate the type of infection and infection rate of human papillomavirus virus typing in high-grade cervical lesions.Methods Statistics different types of HPV infections in 287 patients with cervical highgrade lesions from January 2017 to June 2018 nYangzhou Maternal & Child Care Service Centre.The HPV infection were compared by a single type of HPV infections rates;the different types of HPV infections rates;the HPV infection rates of different age groups;the infection rates of HPV included in different types of vaccines were compared to analyze the possibility of cervical lesions.Results HPV16 infection rate was the highest in single-type HPV infection,131(50.97%),the highest infection rate was observed in different types of HPV infections,159(66.87%),HPV infection of different age groups has the highest infection rate of 30-50 years old,165(64.21%),among the different types of vaccines,the highest rate of infection with HPV type was 237(91.51%).Conclusion HPV infection has a greater impact on high-grade lesions and cervical cancer lesions,the screening of HPV infection has diagnostic significance for cervical cancer screening.The quadrivalent vaccines are not enough to meet the clinical needs,conditional women should receive the nine-valent vaccines as soon as possible.In addition to the subtypes contained in the nine-valent vaccines,the infection rate of other HPV types is also higher,it is suggested that new types should be added to the new vaccines.
