摘要
目的探讨提高血清维生素D水平在预防良性阵发性位置性眩晕(BPPV)复发中的价值。方法将120例于2013年1月-2014年12月期间在宝鸡市中心医院老年病科接受Epley手法复位治疗后的合并维生素D缺乏症的BPPV患者随机分为维生素D治疗组(60例)和无维生素D治疗的对照组(60例),并在开始维生素D治疗后1个月、2个月和6个月检测患者的血清25羟维生素D水平和判断BPPV复发与否,平治疗效果。结果两组患者在接受治疗前的血清25羟维生素D水平无明显差异(10.8±4.3 vs 10.6±4.3,P=0.792)。治疗组患者在接受维生素D治疗2个月后的血清25羟维生素D升高至参考范围(>30 ng·mL-1),并在6个月内维持正常水平,而对照组患者仍处于维生素D缺乏状态。在6个月的随访期内,治疗组患者的复发率仅为3.3%,而对照组患者的复发率达30.0%(P<0.001),两组患者间BPPV复发率的比值比OR=0.080(95%CI:0.017~0.363,P=0.001)。结论提高血清维生素D水平能够有效减少合并维生素D缺乏症的BPPV患者的复发率。
Objective To assess the clinical efficiency of increaseing serum vitamin D levels on preventing the recurrence of benign paroxysmal positional vertigo( BPPV). Method One hundred and twenty cases of BPPV patients complicated with vitamin D deficiency, who were treated with epley maneuver during January 2013 to December 2014 in our department, were randomly divided into two groups based on whether received vitamin D supplemental treatment: vitamin D treatmen group and control group. All patients were followed at 1 month, 2 months and 6 months after vitamin D treatment by detecting serum 25-hydroxy vitamin D( 25-OHD) level and aseessing the recurrence of BPPV. Results The baseline serum 25-OHD level before treatment was similar between two groups (10.8±4.3 vs 10.6±4.3, P=0.792). At month 2 of vitamin D treatment, serum 25-OHD in the treatment group increased significantly to normal level(>30 ng·mL^-1) and maintained in 6 months, whereas serum 25-OHD in the control group remained unchanged. During the 6 months follow up, the recurrence rate of BPPV in treatment group was 3.3%, but pretty higher in control groups( 30.0%). Qualitative variables analysis between two groups confirmed the oddi ratio was( OR=0.080, 95% CI: 0.017-0.363, P=0.001). Conclusion increasing serum vitamin D levels could significantly reduce the recurrence of BPPV.
