摘要
目的系统评价良性阵发性位置性眩晕(Benign paroxysmal positional vertigo,BPPV)患者首次发病的危险因素,为减少BPPV的危险因素暴露、进行早期干预提供循证医学证据。方法系统检索CNKI、VIP、WanFang Data、PubMed、EMbase和Web of Science数据库获得良性阵发性位置性眩晕患者首次发病的危险因素相关的中英文文献。采用Stata 14.0和RevMan 5.3软件进行Meta分析,计算结局指标的计数比(OR)和95%置信区间(CI),采用亚组分析、发表偏倚评价和敏感性分析评价异质性。结果共纳入国内外文献21篇,包含共216416例患者,涉及相关危险因素14个。Meta分析结果显示:女性、高血压、糖尿病、中风、骨质疏松、中耳疾病、年龄、颈椎病、偏头痛、甘油三酯、脑力劳动、过度劳累、维生素D均是BPPV患者首次发病的危险因素。结论女性、高血压、糖尿病等13项危险因素与BPPV首次发病相关。
Objective To identify risk factors for the onset of benign paroxysmal positional vertigo for reducing risk factor exposure and early intervention in BPPV.Methods A systematic search of the CNKI,VIP,WanFang Data,PubMed,EMbase and Web of Science databases was conducted and literatures related to risk factors for onset of benign paroxysmal positional vertigo in both Chinese and English were extracted.Meta-analysis was performed using the Stata 14.0 and RevMan 5.3 software.Odds ratio(OR)and 95%confidence interval(CI)were calculated for outcome indicators,and heterogeneity was evaluated via subgroup analysis,publication bias assessment and sensitivity analysis.Results A total of 21 publications were included,containing a total of 216,416 patients with 14 associated risk factors.Meta-analysis showed that female gender,hypertension,diabetes,stroke,osteoporosis,middle ear diseases,age,cervical spondylosis,migraine,elevated triglycerides,intellectual professions,exhaustion and vitamin D deficiency were correlated to the onset of BPPV.Conclusion Thirteen risk factors,including female gender,hypertension and diabetes,are potential risk factors for the onset of BPPV.
