摘要
目的探讨多轴向旋转座椅红外视频眼震工作系统(G-Force系统)治疗良性阵发性位置性眩晕(BPPV)的效果。方法选取2017年4—12月本院收治的186例BPPV患者作为研究对象,随机分为机器复位组(n=91)和手法复位组(n=95),根据变位性眼震检查结果,判定BPPV类型,机器复位组按不同半规管采用模块化复位,手法复位组后半规管采用Epley管石复位法复位,水平半规管采用Barbecue滚转法复位,比较各耳石类型半规管频数、两组临床疗效、不同类型半规管耳石复发率。结果后半规管共121例,占比65.1%;水平半规管共17例,占比9.1%;前半规管共11例,占比5.9%;多半规管型37例,占比19.9%。机器复位组治疗总有效率为96.7%,手法复位组为91.6%,两组比较差异无统计学意义。随访12个月,患者总复发率为20.4%,多半规管耳石症复发率为45.9%,明显高于单管耳石症的14.1%,差异有统计学意义(P<0.05)。结论运用G-Force系统位治疗BPPV效果确切,与手法复位疗效相当,但应用G-Focre系统可存储患者信息,避免手法复位操作者负重操作,便于改变患者体位,且有助于实现BPPV的标准化诊治,具有较好的临床应用前景。
Objective To investigate the effect of G-Force in the treatment of benign paroxysmal positional vertigo(BPPV).Methods 186 pa-tients with BPPV who were admitted to our hospital from April to December 2017 were selected as the research subjects and they were randomly di-vided into machine reduction group(n=91)and manual reduction group(n=95),for the types of BPPV,the machine reduction group used modular re-duction according to different semicircular canals,the manual reduction group used the Epley canal stone reduction method to reduce the posterior semicircular canals,and the horizontal semicircular canals were reduced by the Barbecue rolling method,the frequency of semicircular canals of each type of otolith,the clinical efficacy of the two groups,and the recurrence rate of different types of semicircular canal otoliths were compared.Results Among the total 189 cases,121 cases were diagnosed as posterior semicircular canal BPPV,accounting for 65.1%;17 cases as lateral semi-circular canal BPPV,accounting for 9.1%;11 cases as superior semicircular cannal BPPV,accounting for 5.9%;37 cases as mutil-canal BPPV,ac-counting for 19.9%.The total effective rate was 96.7%in machine reduction group,while 91.6%in manual reduction group,there was no statistical difference between the two groups.During the 12-month follow-up,the total recurrence rate of patients was 20.4%,and the recurrence rate of multi-semicircular canal otolithiasis was 45.9%,which was significantly higher than 14.1%of single canal otolithiasis,and the difference was statistically significant(P<0.05).Conclusion The G-Force system can be effective in the treatment of BPPV,which is equivalent to manipulation.However,the G-Force system can store patients'information,reduce doctors'physical burden,facilitate the change of patients'position,contribute to the realization of standardized diagnosis and treatment of BPPV,it has good clinical benifit.
