探讨耶鲁-湘雅360度评估体系在住院医师规范化培训中的应用 被引量:9

摘要 本研究旨在探索耶鲁-湘雅360度评估体系在住院医师规范化培训中的作用,为推广有效的临床评估和教学工具提供依据。运用耶鲁-湘雅360度评估体系对住院医师、带教老师、专业基地以及项目主管进行评估。结果显示:2014级住院医师在沟通技巧、诊疗技巧、告知技巧、临床判断能力、临床综合能力这5方面培训第3年的平均得分高于培训第2年,在协作能力方面平均得分有所降低;培训第1年,2016级住院医师的Mini-CEX量表的7项能力均优于2015级住院医师;2016级住院医师的职业道德、职业技能、患者安全、医学伦理、团队精神、创新能力及自我提高能力评分分别高于2015级和2014级,差异均有具统计学意义(P <0.05);2015级住院医师的职业道德、患者安全、团队精神、创新能力及自我提高能力评分高于2014级,差异均具有统计学意义(P <0.05);护士对住院医师的平均好评率达75.24%;患者对住院医师的平均好评率达79.79%;对带教老师、专业基地、项目主管的评估中发现有部分得分较低的项目。培训基地根据评估结果对培训中存在的薄弱环节、不足之处加强训练改进,对成效好的方面继续发扬。应用耶鲁-湘雅360度评估体系能有效提升住院医师6大核心能力。 The purpose of this study is to explore the role of Yale-Xiangya 360-Degree Evaluation System in standardized residency training and provide evidence for popularizing effective clinical evaluation and teaching tools.The system was used to evaluate and analyze residents,teachers,professional bases and project managers.The results showed that the average scores of 2014 grade residents increased in the third year in communication skills,diagnosis and treatment skills,informing skills,clinical judgment and comprehensive abilities compared with the second year of training,and their score decreased in cooperation abilities.In the first year of training,the seven abilities in Mini-CEX of 2016 grade residents were superior to those of residents of 2015 grade.The scores of 2016 residents in professionalism,professional skills,patient safety,medical ethics,team spirits,innovation abilities and self-improvement abilities were higher than those of 2015 and 2014 residents,the difference was statistically significant(p<0.05).The scores of 2015 residents in patient safety,team spirits,innovation abilities and self-improvement abilities were higher than those of the 2014 residents(p<0.05).The average favorable rates of nurses to the resident was 75.24%.The average favorable rate of patients to the residents was 79.79%.Some of the lower scores were found in the assessment of teachers,professional bases and project managers.According to the evaluation results,the training base will strengthen the training and improve the weak links and deficiencies in the training,and carry forward the good results.The Yale-Xiangya 360-Degree Evaluation System can effectively help improve the 6 core competences of residents.
