托拉塞米对AMI伴心力衰竭患者心肾功能的影响 被引量:5

Effect of torasemide on the cardiac and renal function in patients with acute myocardial infarction and heart failure

摘要 目的探讨托拉塞米对急性心肌梗死(AMI)伴心力衰竭患者心肾功能和相关离子水平的影响。方法纳入急性心肌梗死伴心力衰竭患者80例,随机分为对照组、观察组各40例,分别行常规治疗、常规治疗+托拉塞米,比较两组治疗有效率、治疗前后心肾功能、血清N端脑钠肽(NT-pro BNP)、胱抑素C(Cys-C)、24 h尿量、血钠、血钾、血肌酐(Scr)及焦虑评分(SAS)、6 min步行试验(6MWT)结果、不良反应。结果观察组治疗有效率92.50%,高于对照组75.00%,差异有统计学意义(P<0.05);观察组治疗后左室射血分数(LVEF)、心功能分级Ⅰ~Ⅱ级比例、24 h尿量、血钠、血钾及6MWT高于对照组,而血清Scr、NT-pro BNP、Cys-C、SAS评分及不良反应发生率低于对照组,差异均有统计学意义(P<0.05)。结论托拉塞米可明显改善AMI并心衰患者心肾功能,降低相关离子水平,减少不良反应,值得在临床推广应用。 Objective To investigate the effect of torasemide on the cardiac and renal function and levels of related ions in patients with acute myocardial infarction(AMI)and heart failure(HF).Methods 80 patients with AMI and HF were randomly divided into the control group and the observation group,40 cases in each group.The control group and observation group were given routine treatment and routine treatment combined with torasemide treatment,respectively.The effective rate of treatment,cardiac and renal function,serum N terminal brain natriuretic peptide(NT-pro BNP),cysteine C(Cys-C),24 h urine volume,serum sodium,serum potassium,serum creatinine(Scr),the Self-rating Anxiety Scale(SAS)score,results of 6 min walking test(6 MWT)and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(92.50% vs.75.00%)(P〈0.05).After treatment,the left ventricular ejection fractions(LVEF),the proportion of grade Ⅰ~Ⅱ cardiac function,24 h urine volume,serum sodium,serum potassium and 6 MWT in the observation group were significantly higher than those in the control group,while serum Scr,NT-pro BNP,Cys-C,SAS score and the incidence of adverse reactions were lower than those in the control group(P〈0.05).Conclusion Torasemide could significantly improve the renal function in patients with AMI and HF,and reduce the levels of related ions and adverse reactions.
