摘要
目的探讨托拉塞米治疗伴利尿剂抵抗糖尿病肾病患者的临床效果。方法选取2017年1月~2018年12月我院收治的46例伴利尿剂抵抗的糖尿病肾病患者作为研究对象,按照电脑随机分配的方法分为对照组(23例)和观察组(23例)。对照组给予呋塞米治疗,观察组给予托拉塞米治疗。比较两组治疗前后的24 h尿量、体重、血肌酐水平及低血钾症发生率。结果治疗前,两组的24 h尿量、体重及血肌酐水平比较,差异无统计学意义(P>0.05);治疗后7 d,两组的体重和血肌酐水平比较,差异无统计学意义(P>0.05);治疗后1、3、7 d,两组的24 h尿量高于治疗前,观察组的24 h尿量高于对照组,差异均有统计学意义(P<0.05);观察组治疗后的低钾血症发生率为4.35%,低于对照组的26.09%,差异有统计学意义(P<0.05)。结论托拉塞米和呋塞米治疗伴利尿剂抵抗的糖尿病肾病患者的效果相当,托拉塞米能改善患者的水肿,降低低钾血症的发生率,值得临床推广应用。
Objective To investigate the clinical effect of Tolasemide in the treatment of diabetic nephropathy with diuretic resistance.Methods A total of 46 patients with diabetic nephropathy and diuretic resistance admitted to our hospital from January 2017 to December 2018 were selected as the study subjects,they were divided into control group(23 cases)and observation group(23 cases)according to the method of computer random allocation.The control group was treated with Furosemide and the observation group was treated with Torasemide.The urine volume of 24 hours,body weight,serum creatinine level before and after treatment and incidence of hypokalemia were compared between the two groups.Results Before treatment,there were no significant differences in 24-hour urine volume,body weight and serum creatinine level between the two groups(P>0.05).After 7 days of treatment,there were no significant differences in body weight and serum creatinine levels between the two groups(P>0.05).After 1,3 and 7 days of treatment,the 24-hour urine volume of the two groups was higher than that before treatment,the 24-hour urine volume of the observation group was higher than that of the control group,the differences were statistically significant(P<0.05).The incidence of hypokalemia in the observation group was 4.35%,which was lower than 26.09%in the control group,the difference was statistically significant(P<0.05).Conclusion Tolasemide and Furosemide have similar effects in the treatment of diabetic nephropathy patients with diuretic resistance.Tolasemide can improve edema and reduce the incidence of hypokalemia,which is worthy of clinical promotion and application.
