摘要
目的 探讨慢性心力衰竭患者血清微小RNA-423-5p(miR-423-5p)的表达水平及其意义.方法 选择2015年7月至2017年3月我院收治的89例慢性心力衰竭患者为病例组,并于同期随机选取35例健康体检者为对照组,采用实时荧光定量PCR(RT-PCR)法对两组受试者血清miR-423-5p水平进行检测.结果 病例组血清miR-423-5P水平为(0.97±0.32),高于对照组的(0.41±0.28),差异有统计学意义(t=9.072,P<0.01).慢性心力衰竭miR-423-5p高表达患者血清NT-proBNP、LVEDD水平高于miR-423-5p低表达患者,LVEF低于miR-423-5p低表达患者,心功能分级高于miR-423-5p低表达患者,差异均有统计学意义(P<0.05).MiR-423-5p诊断慢性心力衰竭的灵敏度为89.92%,特异度为93.34%,ROC曲线下面积AUC为0.919(95%CI0.862~0.975).结论 MiR-423-5p在慢性心力衰竭患者血清中异常表达,并且与心功能密切相关,早期检测可作为评估心力衰竭严重程度、诊断慢性心力衰竭的重要生物学标志物.
Objective To explore the expression of serum microRNA-423-5 (miR-423-5p) in patients with chronic heart failure and its clinical significance.Methods A total of 89 patients with chronic heart failure in our hospital from July 2015 to Marh 2017 as case group were enrolled,at the same time,a total of 35 healthy volunteers as control group were randomly selected.The serum miR-423-5p level of two groups was tested by the real-time quantitative PCR (RT-PCR).Results The serum miR-423-5p level of case group was (0.97±0.32),which was higher than that of control group (0.41±0.28),the difference was statistically significant (t=9.072,P〈0.001).The serum NT-proBNP,LVEDD level in patients with high expression of miR-423-5p were higher than those in patients with low expression of miR-423-5p,and the LVEF level was lower than that in patients with low expression of miR-423-5p,and the classification of cardiac function also was higher than that in patients with low expression of miR-423-5p,the difference was statistically significant (P〈0.05).The sensitivity of miR-423-5p in the diagnosis of chronic heart failure was 89.92%,the specificity was 93.34%,and the AUC of ROC curve was 0.919 (95%CI 0.862-0.975).Conclusion The serum miR-423-5p in patients with chronic heart failure abnormally express,which is closely related to heart function.Early detection can be used as an important bio-marker for assessing the severity of heart failure and diagnosing chronic heart failure.
