摘要
目的观察苓桂兰草汤联合左西孟旦注射液对急性心力衰竭(AHF)水饮内停证患者微小RNA-1(miR-1)、miR-181b表达的影响。方法将124例AHF水饮内停证患者按照随机数字表法分为2组。对照组62例予左西孟旦注射液治疗,治疗组62例在对照组治疗基础上联合苓桂兰草汤治疗,2组均治疗14 d。比较2组治疗前后中医证候评分、左室舒张末期容积指数(LVEDVI)、左室收缩末期容积指数(LVESVI)、左室质量指数(LVMI)、血清超敏肌钙蛋白I(hs-cTnI)、肌酸激酶同工酶(CK-MB)、肿瘤坏死因子α(TNF-α)、白细胞介素17(IL-17)及miR-1、miR-181b表达,并统计2组疗效。结果治疗组总有效率95.16%(59/62),对照组总有效率82.26%(51/62),治疗组疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后LVEDVI、LVESVI均较本组治疗前降低(P<0.05),LVMI均升高(P<0.05),且治疗组治疗后LVEDVI、LVESVI均低于对照组(P<0.05),LVMI高于对照组(P<0.05)。2组治疗后hs-cTnI、CK-MB水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后TNF-α、IL-17水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后miR-1、miR-181b表达均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。结论苓桂兰草汤联合左西孟旦注射液治疗AHF水饮内停证可有效调控miR-1、miR-181b表达,改善中医证候,延缓心室重构,减轻心肌损伤、炎性反应程度,疗效显著。
Objective To observe the effects of Linggui Lancao Decoction with levosimendan injection on microRNA-1(miR-1)and microRNA-181b(miR-181b)in patients with acute heart failure(AHF)of internal retention of water syndrome.Methods Totally 124 AHF patients(internal retention of water syndrome)were randomly assigned to two groups,patients in treatment group(n=62)were treated with leosimendan injection with Linggui Lancao Decoction,and patients in control group(n=62)with leosimendan injection only.The treatment period was 14 days,traditional Chinese medicine syndrome(TCM)score,left ventricle end-diastolic volume index(LVEDVI),left ventricular end-systolic volume index(LVESVI),left ventricular mass index(LVMI),serum high-sensitivity cardiac troponin I(hs-cTnI),creatine kinase isoenzyme(CK-MB),tumor necrosis factor-α(TNF-α),interlukin-17(IL-17),expression of miR-1 and miR-181b were included as comparators;the curative effect was counted.Results The overall effective rate was 95.16%(59/62)in treatment group,higher than that of 82.26%(51/62)in control group(P<0.05).After treatment,TCM syndrome scores in groups were decreased(P<0.05),which were decreased in treatment group than in control group(P<0.05);LVEDVI and LVESVI in groups were decreased(P<0.05),treatment group was lower than control group(P<0.05),LVMI in groups was increased(P<0.05),treatment group was more prevalent(P<0.05);hs-cTnI and CK-MB in groups were decreased(P<0.05),which were lower in treatment group than in control group(P<0.05);TNF-αand IL-17 in groups were decreased,which was more common in treatment group(P<0.05);the expression of miR-1 and miR-181b in groups was elevated(P<0.05),and improvement was more in treatment group(P<0.05).Conclusion Linggui Lancao Decoction with levosimendan injection can efficiently regulate the expression of miR-1 and miR-181b on AHF patients(internal retention of water syndrome),which can improve TCM syndromes,delay ventricular remodeling,reduce myocardial injury and inflammatory response,with significant curative effect.
