摘要
目的观察中医推拿联合自拟四君子汤直肠滴注治疗小儿β-内酰胺类抗菌药物相关性腹泻的疗效及对肠道菌群和免疫功能的影响。方法将94例β-内酰胺类抗菌药物相关性腹泻患儿随机分为2组,对照组47例给予常规西医治疗,观察组47例在对照组治疗基础上给予中医推拿联合自拟四君子汤直肠滴注治疗,疗程均为7 d。观察2组临床症状改善情况及临床结局,检测2组治疗前后肠道菌群及免疫功能相关指标情况。结果治疗后2组大便频率、大便性状、腹痛、腹胀、呕吐、食量减少、口干积分均明显降低(P均<0.05),且观察组上述积分均显著低于对照组(P均<0.05);观察组腹泻缓解时间、呕吐缓解时间、腹痛缓解时间、腹胀缓解时间、便常规恢复正常时间、住院时间均明显短于对照组(P均<0.05);治疗后2组粪便球/杆菌比例、s IgA含量及血清二胺氧化酶、降钙素原(PCT)、IgG、IgA、CD4^+、CD4^+/CD8^+均显著改善(P均<0.05),且观察组各指标改善情况均显著优于对照组(P均<0.05)。结论中医推拿联合自拟四君子汤直肠滴注能够显著改善β-内酰胺类抗菌药物相关性腹泻患儿的临床症状,促进病情恢复及缩短住院时间,其机制可能与改善患儿免疫功能并促进患儿肠道菌群平衡有关。
Objective It is to observe the effect of massage combined with Sijunzi Decoction by rectum drip on infantile diarrhea induced by β-Lactam antibiotics, and the effect on intestinal flora and immune function. Methods 94 cases of children with diarrhea induced by β-Lactam antibiotics were randomly divided into 2 groups： the control group （n = 47） was treated with normal western medical therapy, the observation group （ n = 47） was treated with massage combined with Sijunzi Decoction by rectum drip, both groups were treated for 7 days. The improvements of clinical symptoms and clinical outcomes of the two groups were observed, and the related indexes of intestinal flora and immune function before and after treatment were detected. Results After treatment, the scores of defecation frequency, fecal behavior, abdominal pain, abdominal distension, vomiting, decreased food intake, and dry were all significantly reduced, and the above scores in the observation group were all lower than those in the control group （P 〈 0.05 ）. The relief times of diarrhea, vomiting, abdominal pain, abdominal distension and remission time of stool routine and hospitalization time were significantly shorter than those in the control group （P 〈 0.05 ）. After treatment, slgA content and ratio between stool ball/coil, serum procaichonin （PCT）, IgG, IgA and CD4^＋ , CD4^＋/CD8^＋ were significantly improved （P 〈 0.05 ）, the improvements of these indexes in the observation group were better than that of control group （ P 〈 0.05 ）. Conclusion TCM massage combined with Sijunzi decoction by rectum drip can significantly improve the clinical symptoms of infantile diarrhea induced by β-Lactam antibiotics, promote the recovery of body and shorten the length of hospital stay, the mechanism may be related to improve immune function, and promote intestinal flora balance in the children.
