中性粒细胞表面抗原CD64指数和降钙素原在广谱抗生素致儿童肠道菌群失调性腹泻中的诊断价值 被引量:6

摘要 目的探讨CD64、PCT在广谱抗生素致儿童肠道菌群失调性腹泻中的诊断价值。方法收集2016年1月-6月在本院住院治疗的广谱抗生素致肠道菌群失调性腹泻患儿72例作为研究对象,同时选取50例健康体检儿童作为正常对照组,分析患儿治疗前后CD64、PCT水平变化,并对CD64、PCT的诊断效率和相关性进行分析。结果 72例广谱抗生素致肠道菌群失调性腹泻患儿治疗前CD64、PCT水平明显高于正常对照组,经过停用广谱抗生素,使用窄谱抗生素或应用微生态制剂治疗后CD64、PCT水平明显下降,与治疗前比较,差异有统计学意义(P<0.01)。CD64、PCT在广谱抗生素致儿童肠道菌群失调性腹泻中阳性率、敏感度、特异度、阳性预测值和阴性预测值均>80.00%,治疗前CD64的表达与PCT水平呈正相关(r=0.865,P<0.01)。结论临床应重视抗生素可致肠道内微生态失调,联合检测CD64、PCT对广谱抗生素致儿童肠道菌群失调性腹泻的早期诊断、预后判断和疗效观察具有一定的指导意义。 Objective To explore the value of peripheral blood neutrophil surface antigen CD64 index and procalcitonin( PCT)in the diagnosis of intestinal flora imbalance diarrhea caused by broad spectrum antibiotics in children. Methods 72 cases of intestinal flora imbalance diarrhea during January-June 2016 in the hospital were selected for the treatment of broad spectrum antibiotic,at the same time,50 healthy children were selected as the normal control group to analyze the changes of CD64 and PCT levels before and after treatment,and the diagnostic efficiency and correlation of CD64 and PCT were analyzed. Results The levels of CD64 and PCT were significantly higher than those of normal control group before the treatment in 72 patients with intestinal flora imbalance diarrhea caused by broad spectrum antibiotic,after discontinuation of broad-spectrum antibiotics,the levels of PCT and CD64 significantly decreased after treatment with narrow spectrum antibiotics and probiotics,with the difference statistically significant than that before treatment( P 0. 01). The positive rate,sensitivity,specificity,positive predictive value and negative predictive value of PCT and CD64 in a broad spectrum of antibiotics in children with intestinal flora imbalance diarrhea were all 80. 00%,and the expression of CD64 was positively correlated with the PCT level before treatment( r = 0. 865,P 0. 01). Conclusion Clinical attention should be paid to that antibiotics can lead to intestinal micro ecological imbalance,combined detection of CD64 and PCT has certain guiding significance in the early diagnosis,prognosis and curative effect observation of intestinal flora imbalance diarrhea caused by broad spectrum antibiotics in children.
