磁共振成像引导下射频消融术对乳腺癌的临床研究 被引量:4

摘要 目的探讨磁共振成像(MRI)引导下射频消融术对乳腺癌的临床应用。方法 11例经穿刺病理确诊的晚期乳腺癌患者,经4~6周期化疗或内分泌治疗及靶向治疗等综合治疗后乳腺残留病灶难以进一步缩小,影像学检查乳腺仍残存病灶,再次穿刺病理活检证实有活性肿瘤组织细胞残存。随机对乳腺残存病灶行射频消融治疗。术后3、6、12个月进行临床及影像学随访。结果本组11例患者都顺利完成射频消融治疗,仅1例发生皮肤局部烧伤。MRI检查示肿瘤病灶均发生坏死,瘤内血供消失,无强化区。皮肤烧伤患者,经对症处理,2个月后烧伤痊愈。1年后检查显示乳腺病灶模糊或者消失。乳腺病灶全部达到完全缓解,有效率为100%。术后随访12~24个月所有患者均生存。结论 MRI引导下射频消融术对乳腺癌安全有效。 Objective To explore the clinical application of MRI-guided radiofrequency ablation (UFA) in breast cancer. Method Eleven patients with core-needle biopsy-proven advanced breast carcinoma were enrolled in this study.Moreover, all of the tumor size could not be further shrunk after medical treatment (such as 4-6 cycles chemotherapy, endocrine therapy or targeted therapy). Additionally, breast lesions were still remained to be confirmed by imaging examination and biopsy pathology. Then the tumor and surrounding breast tissue were ablated with ra-diofrequency, followed by clinical follow-up and imaging examination after 3,6 and 12 months. Results All patients completed RFA treatment, 1 case suffered local skin bums. MRI showed that all the tumor lesions were necrotic, blood supply was disappeared and there was no enhancement. Skin bum of the patient was treated, after 2 months, the bum healing. After 1 year, radiographic examination showed the original breast lesions were fuzzy or disappeared. All tumors were achieved complete remission (CR) examined by imaging, and the effective rate was 100%. All postopera-tive patients survived 1 year according to the follow-up data. Conclusions MRI-guided radiofrequency ablation is safe and effective method in breast cancer.【
