摘要 [目的]探讨Orem自护模式理论在鼻咽癌放疗病人鼻腔冲洗中的应用效果。[方法]将100例鼻咽癌放疗病人采用随机数字表法分为对照组和观察组各50例,对照组采用常规护理,观察组应用完全补偿系统护理、部分补偿系统护理、支持-教育系统护理,采用自行设计舒适度评分表、焦虑自评量表(SAS)、睡眠质量指数量表(PSQI)、美国放射治疗协作组织(RTOG)分级标准评价两组病人的舒适度、心理情况、睡眠质量及鼻咽部黏膜反应症状程度,并对两组病人的护理效果进行比较。[结果]干预后观察组病人对鼻腔冲洗的掌握及坚持冲洗情况明显优于对照组,舒适度评分明显高于对照组,SAS评分、PSQI评分明显低于对照组,鼻腔放射性黏膜反应发生率明显低于对照组,经比较差异均有统计学意义(P<0.05)。[结论]应用Orem自理模式理论在鼻咽癌放疗病人的鼻腔冲洗中,可强化病人自我护理意识,使病人正确掌握鼻腔冲洗的方法,保持鼻咽部清洁通畅,提高舒适度,减轻焦虑,促进睡眠,减轻鼻咽黏膜反应。 Objective： To probe into the application effect of Orem self - care model theory in nasal washing ofnasopharyngeal carcinoma patients undergoing radiotherapy.Methods ： A total of 100 patients with nasopharyn-geal carcinoma were randomly divided into control group and observation group with random number tablemethod,50 cases in each.The patients in control group were treated with routine nursing.The patients in obse--vation group were treated with complete compensation system nursing and partial compensation system nurs-ing.The support education system nursing （SAS） ,Sleep Quality Index （PSQI） , American Society of Radiation Therapy （RTOG） grading criteria were used to evaluate the comfort, psychological condition, quality of sleep and the nasopharyngeal mucosal response symptoms of the patients in the two groups.And the nursing effect of patients was compared between both groups.Results： After the intervention,the effect of patients mastering and adhering to nasal washing were better than that in control group, the comfort scothan that in control group,SAS score,PSQI score were significantly lower than those in dence of nasal radioactive mucosal reaction was significantly lower than that in control grstatistically significant （P 〈0.05）.Conclusion： Oremself - care model should be used to strengthen the patient’sself-care consciousness in the nasal washing of patients with nasopharyngeal carcinoma radiotherapy ？ so that the patients can master the nasal washing method, keep the nasopharynx clean and smooth, improve the com-fort, reduce anxiety and promote sleep and reduce nasopharyngeal mucosal response.
