摘要
目的 探讨无肌松药下靶控输注丙泊酚复合舒芬太尼在妇科腹腔镜手术中的麻醉效果.方法 将2015年5月至2016年5月择期全身麻醉下行妇科腹腔镜的96例患者根据术中是否使用肌松药随机分为观察组和对照组,每组48例,观察组在靶控输注舒芬太尼和丙泊酚且不使用肌松药维持麻醉,而对照组在靶控输注舒芬太尼和丙泊酚维持麻醉下间断追加苯磺顺阿曲库铵,比较两组患者在不同时间点的血流动力学指标以及麻醉恢复的相关指标.结果 观察组拔管时收缩压、舒张压及心率均低于对照组,差异有统计学意义（P＜0.05）,两组在其余时间点比较差异未见统计学意义（P0.05）.观察组自主呼吸恢复时间、呼唤睁眼时间、拔管时间以及离室时间均短于对照组,而拔管时BIS值则高于观察组,差异均有统计学意义（P＜0.05）.结论 无肌松药下靶控输注丙泊酚复合舒芬太尼麻醉在妇科腹腔镜手术中安全可行,且术后苏醒迅速,麻醉恢复期风险低.
Objective To investigate the feasibility of gynecologic laparoscopy surgery without muscle relaxant after anesthesia with target controlled propofol sufentanil infusion.Methods Ninety-six cases of gynecological laparoscopic surgery under general anesthesia from May 2015 to May 2016 were randomly divided into observation group （n=48） and control group （n=48） according to with muscle relaxant or not. The patients in observation group were treated with target controlled propofol sufentanil infusion without muscle relaxant, and the patients in control group were treated with target controlled propofol sufentanil andcisatracurium besilate infusion. The hemodynamic indexes at different time points and the related indexes of the recovery of anesthesia of two groups were compared.Results The systolic blood pressure, diastolic blood pressure and heart rate in observation group at tracheal extubation were lower than those in control group, and the differences were significant（P〈0.05）. There were no significant differences for other time points between the two groups（P〉0.05）. The recovery time of spontaneous breathing, call for the opening time, extubation time and the time of leaving room in observation group were shorter than those in control group, and the differences were significant（P〈0.05）.Conclusions Gynecologic laparoscopy surgery without muscle relaxant after anesthesia with target controlled propofol sufentanil infusion is safe and feasible, while had quickly regained consciousness after the surgery and low risk of anesthesia recovery.
