摘要
目的:分析米非司酮配伍妈富隆治疗药物流产不全的临床疗效及安全性。方法:选取2015年6月至2016年6月收治的药物流产不全患者106例,随机将其分为观察组和对照组,每组各53例;对照组行常规清宫术,观察组给予口服米非司酮配伍妈富隆。观察两组的阴道流血时间、尿妊娠试验转阴时间、月经复潮时间及月经复潮量,评价两组的临床疗效及并发症,并观察治疗期间出现的不良反应。结果:观察组与对照组治愈率分别为98.1%、92.5%,两组比较差异无统计学意义(P>0.05);观察组盆腔炎的发生率为1.9%,宫腔/宫颈粘连的发生率为1.9%均低于对照组的17.0%、20.8%(P<0.05);观察组月经复潮时间短于对照组(P<0.05),月经干净后子宫内膜厚度高于对照组(P<0.05);两组治疗期间血尿常规及心、肝、肾功能均在正常范围内,两组不良反应情况比较差异无统计学意义(P>0.05)。结论:米非司酮联合妈富隆和清宫术治疗药物流产不全均安全有效,但米非司酮联合妈富隆保守治疗的并发症发生率低,且可缩短月经复潮时间。
Objective： To discuss the clinical effects and safety of mifepristone combined with marvelon in the treatment of incomplete drug abortion. Methods：106 cases of patients with incomplete medical abortion from January 2013 to December 2014 in the hospital were selected and divided into observation group and control group（ 53 cases in each group） in accordance with the random number method. The patients in control group were given routine complete curettage of uterine cavity and those in observation group received oral mifepristone combined with marvelon. The vaginal bleeding duration,time of turning negative in urine pregnancy test,and time and amount of menstruation recovery were observed. Clinical efficacy,complication and adverse reactions in the two groups were recorded and compared. Results： There was no significant difference of cure rates in observation group（ 92. 4 %） and control group（ 98. 1 %）（ P〈0. 05）. The incidences of pelvic inflammatory disease（ 1. 9 %） and cervical adhesion（ 1. 9 %） in observation group were significant lower than those in control group（ 17. 0 % and 20. 7 % respectively; P〉0. 05）. The menstruation recovery time in observation group was significantly lower than that in control group（ P〈0. 05）. Endometrial thickness after menstruation in observation group was significantly higher than that in control group（ P〈0. 05）. There was no abnormality in terms of routine blood test,heart,liver and kidney function in the two groups and the adverse reactions between the two groups had no statistical significance（ P〉0. 05）. Conclusions： Both mifepristone combined with marvelon and curettage of uterine cavity in the treatment of incomplete drug abortion are effective and safe,but the conservative treatment of mifepristone combined with marvelon has lower complication incidence and lower menstruation recovery time.
