超声弹性成像与常规超声诊断乳腺癌腋淋巴结转移 被引量:20

Ultrasound elastography and conventional ultrasound in diagnosis of metastatic axillary lymph nodes in patients with breast cancer

摘要 目的比较采用常规超声(US)与超声弹性成像(UE)判断腋淋巴结性质的价值。方法观察我院60例经病理证实为乳腺癌患者的84枚腋淋巴结,均行US及UE检查,以病理结果为金标准,构建ROC曲线,比较US、UE及US结合UE(US+UE)诊断腋淋巴结性质的效能。结果应用US、UE及US+UE判断乳腺癌腋淋巴结转移的ROC曲线下面积分别为0.782、0.812、0.858。UE与US曲线下面积的差异无统计学意义(Z=1.613,P>0.05)。US+UE与US(Z=2.131,P<0.05)、与UE(Z=1.996,P<0.05)的差异均有统计学意义。结论联合应用常规US与UE可以提高诊断转性性腋淋巴结的效能。 Objective To compare the value of ultrasound elastography(UE)versus conventional ultrasound(US)in diagnosis of metastatic axillary lymph nodes in patients with breast cancer.Methods A total of 60 patients with 84 axillary lymph nodes confirmed by pathology were included in the study.Conventional US and UE were performed on each patient.With pathology as golden standard,the diagnostic efficiency among conventional US,UE and conventional US combined with UE(US+UE)were compared using ROC curve.Results The area under the ROC curve(AUC)of conventional US,UE and US+UE was 0.782、0.812 and 0.858,respectively.The differences of AUC between US+UE and conventional US(Z=2.131,P〈0.05)or UE(Z=1.996,P〈0.05)were statistically significant,while no significant differences were shown between conventional US and UE(Z=1.613,P〉0.05).Conclusion Conventional US combined with UE may be helpful to increase the diagnostic efficiency of metastatic axillary lymph nodes in patients with breast cancer.
