妊娠期肝内胆汁淤积症患者胎盘线粒体呼吸链酶活性和端粒酶反转录酶改变的研究 被引量:2

Expression of mitochondrial respiratory chain enzymes and human telomerase reverse transcriptase in placental trophocytes in patients with intrahepatic cholestasis of pregnancy

摘要 目的研究胎盘滋养细胞线粒体呼吸链酶活性及端粒酶反转录酶(hTERT)的表达与妊娠期肝内胆汁淤积症(ICP)的关系。方法选取27例ICP患者(ICP组)及18例正常孕妇(对照组),采用酶组织化学法分析各组胎盘滋养细胞中线粒体呼吸链酶活性,采用免疫组织化学方法检测各组胎盘滋养细胞中hTERT的表达情况,采用实时荧光定量聚合酶链反应定量检测各组胎盘中hTERTmRNA的表达情况。结果细胞色素c氧化酶、三磷酸腺苷酶和琥珀酸脱氢酶在ICP组(包括ICP轻度亚组及ICP重度亚组)和对照组胎盘滋养细胞中的表达均呈阳性,ICP组上述酶活性均显著低于对照组(P值分别<0.05、0.01)。hTERT在ICP组(112.67±41.40)、ICP轻度亚组(109.56±42.66)及ICP重度亚组(118.89±40.46)胎盘滋养细胞中的表达水平均显著高于对照组(29.06±14.30,P值均<0.05)。ICP轻度亚组与ICP重度亚组间的线粒体呼吸链酶活性和hTERT表达水平的差异均无统计学意义(P值均>0.05)。ICP组、ICP轻度亚组、ICP重度亚组及对照组hTERTmRNA的表达水平分别为3.10±0.12、3.32±0.13、2.66±0.22、2.76±0.08,18srRNA的表达水平分别为0.09±0.11、0.13±0.14、0.23±0.24、0.10±0.11,各组间hTERTmRNA和18srRNA表达水平的差异均无统计学意义(P值均>0.05)。结论 ICP患者胎盘绒毛受损,滋养细胞线粒体呼吸链酶活性下降,hTERT蛋白表达的增加无法弥补增多的细胞凋亡,机体失代偿,导致胎盘功能下降,这可能是ICP胎儿宫内缺氧的一种发生机制。 [Abstract] Objective To investigate the correlation between intrahepatic cholestasis of pregnancy (lOP) and the expression of mitochondrial respiratory chain enzymes and human telomerase reverse transcriptase (hTERT) in placental trophocytes. Methods Twenty-seven lOP patients (lOP group) and eighteen normal pregnant women (control group) were enrolled in the present study. The activity of mitochondrial respiratory chain enzymes in placental trophocytes was examined by enzyme-histochemical method. The expression of hTERT protein was measured by immunohistochemical method. The level of hTERT mRNA was measured by real-time quantitative reverse transcription polymerase chain reaction. Results The expression of cytochrome c oxidase (COO), adenosine triphosphatase (ATPase) and succinate dehydrogenase (SDH) in the lOP group were significantly lower than those in the control group (P〈O. 05 and 0.01). The levels of hTERT in lOP group, mild lOP subgroup, severe lOP subgroup and control group were 112.67 4- 41.40, 109.56 4- 42.66, 118.89 4- 40.46 and 29.06 4- 14.30, respectively. There was significant difference in hTERT between lOP groups and control group (all P〈0.05). There were no significant differences in the expression of mitochondrial respiratory chain enzymes or hTERT between mild and severe lOP subgroups (both P〉0.05). The levels of hTERT mRNA in the above groups were 3.10±0. 12, 3.32 ± 0. 13, 2.66± 0.22 and 2.76± 0.08, respectively. The levels of 18srRNA in these groups were 0.09± 0. 11, 0. 13 ±0. 14, 0.23 ± 0.24 and 0. 10±0.11, respectively. There were no significant differences in hTERT mRNA or 18srRNA between ICP groups and control group (all P〉0.05). Conclusion The placental villi are impaired and the activity of mitochondrial respiratory chain enzymes is decreased in lOP patients. Although the expression of hTERT protein is increased, it can net protect cells from apoptosis and maintain the placental function. It may be a possible mechanism of fetal hypoxia in ICP.
