多重耐药鲍曼不动杆菌DNA同源性与耐药性分析 被引量:2

Analysis on DNA Homology and Drug Resistance of Multidrug-resistant Acinetobacter baumannii

摘要 目的分析邵阳市中心医院多重耐药鲍曼不动杆菌菌株DNA同源性与耐药性,了解多重耐药鲍曼不动杆菌在该院的流行状况。方法 DNA同源性分析:收集该院2009年1-6月份临床送检标本分离的40株多重耐药鲍曼不动杆菌,采用肠杆菌科间一致重复序列PCR方法;药敏试验:采用VITEK-2细菌鉴定仪器法与KB法确定药敏结果。结果 DNA同源性分析结果:40株多重耐药鲍曼不动杆菌共检测出4种电泳条带数目位置完全相同的克隆株,其中A型11株,B型11株,C型9株,D型9株。菌株分布在该院外科、内科I、CU、新生儿科四个病区,10个科室。普通病室与ICU病室均存在A、B、C、D 4种型别克隆株感染。药敏试验结果:头孢哌酮/舒巴坦耐药率仅为5%,4种克隆株均对头孢哌酮/舒巴坦较敏感;阿米卡星耐药率为17.5%,A、B、C型克隆株对阿米卡星较敏感,但D型克隆株对阿米卡星的敏感性明显降低;4种克隆株对亚胺培南耐药率已达75.0%,显著高于文献报道的耐药率。4种克隆株对庆大霉素、氨苄西林/舒巴坦、哌拉西林/他唑巴坦、头孢他啶、头孢吡肟、环丙沙星、左氧氟沙星7种抗生素耐药率均达60%以上,较多克隆株的耐药率已达100%。结论 A、B、C、D 4种克隆株在该院未见明显集中流行趋势,呈散在分布特点。4种克隆株检出率差异无统计学意义。 Objective To analyze the drug resistance and homology of multi - drug resistant Acinetobacter baumannii in the Central Hospital of Shaoyang City, and to investigate the prevalence of the bacterium in the hospital. Methods Forty strains of multidrug - resistant Acinetobacter baumannii were collected from clinical samples in the Central Hospital of Shaoyang City from January to June 2009. Enterobacterial repetitive intergenic consensus (ERIC) was used to analyze DNA homology of these isolates. Antimicrobial susceptibility test was performed by both Vitek - 2 compact automatic system and disk diffusion. Results Among forty strains of multi - drug resistant Acinetobacter baumannii, four types of cloning strains whose electro- phoresis strips had the same number and position were detected, including 11 strains with type A, 11 strains with type B, 9 strains with type C and 9 strains with type D. These isolates were collected from department of surgery, department of internal medicine, ICU, neonatal department, etc. There were cloning strains with types A, B, C, and D in both common wards and ICU. The susceptibility rate of cefoperazone/sulbactam was only 5 %. All four types of cloning strains were sensitive to cefopera- zone/sulbactam. The resistance rate of the strains to amikacin was 17.5 %. Cloning strains with types A, B and C were suscepti- ble to amikacin, but the susceptibility rate of amikacin against type D was significantly decreased. The resistance rate of four types of cloning strains to imipenem was 75 %, which was significantly higher than the relative reference. The resistance rates of four types of cloning strains to gentamycin, ampicillin/sulbactam, piperacillin]tazobactam, ceftazidime, cefepime, ciprofloxacin, and levofloxacin were all over 60%, moreover, many resistant strains had reached 100%. Conclusions The cloning strains with types A, B, C, and D took on a scattering distribution in the hospital, and no prevalent trend was found. There was no sta- tistically significant difference among the detection rates of the four types.
