摘要 目的探讨孕妇妊娠晚期B族溶血性链球菌(GBS)带菌状况以及对妊娠结局及新生儿的影响。方法2005年12月—2007年3月在上海交通大学医学院附属国际和平妇幼保健院产科门诊行常规产检的992名孕妇,于孕34~37周在阴道下段及肛周采样,血琼脂平板培养基培养。33例GBS阳性患者为观察组,进行1∶2配比病例对照研究,66例GBS阴性患者为对照组,并随访孕妇胎膜早破、早产、新生儿感染等情况。结果上海地区孕妇妊娠晚期的GBS带菌率为3.7%(37/992)。观察组孕前阴道炎的患病率为30.3%(10/33),显著高于对照组的6.1%(4/66,χ2=8.746,P=0.003)。观察组的孕妇胎膜早破率为12.1%(4/33),与对照组7.6%(5/66)的差异无统计学意义(χ2=0.138,P=0.711)。观察组中,新生儿发热2例,患病率为6.1%(2/33),略高于对照组的4.6%(3/66),但两组间的差异无统计学意义(χ2=0.000,P=1.000)。两组均无早产发生。结论观察组孕前阴道炎患病率高于对照组;孕妇GBS感染并非胎膜早破的直接原因。 Objective To investigate the group B streptococcus (GBS) colonization rate in pregnant women during late pregnancy and its influence on the pregnant outcomes. Methods Blood agar culture medium was used for culture of GBS in the blood samples obtained from distal vaginal segment and around the recta of pregnant women at gestation age 34-37 weeks, who underwent routine pregnant examination in our hospital from Dec. 2005 to Mar. 2007. A 1 : 2 matching case control study was carried out with 33 pregnant women positive of GBS and 66 negative of GBS. The complications, such as premature rupture of membranes, premature labor, and infection of infants, etc. were followed up. Results The late pregnancy GBS colonization rate in 992 pregnant women was 3.7% (37/992). The colpitis rate was 30.3% (10/33)in pregnant women before pregnancy in the study group and 6. 1% (4/66) in the control group ( χ^2= 8. 746, P = 0. 003). The rate of premature rupture of membranes was 12.1% (4/33) in the study group and 7.6% (5/66)in the control group, with no significant difference between the two groups(χ^2 = 0. 138, P=0.711 ). There was no premature labor in both groups. Fever occurred in 2 newborns in the study group(2/33) and in 3 in the control group (3/66), with no significant difference between two groups(χ^2 = 0. 000, P = 1. 000). Conclusion The rate of colpitis of pre-pregnancy in study group is higher than that in the control group. Colonization of GBS is not the direct reason of premature rupture of membranes.
