经皮神经电刺激联合康复机器人对脑卒中后肢体功能障碍患者步态及平衡水平的影响

Effects of Percutaneous Electrical Nerve Stimulation Combined with Rehabilitation Robot on Gait and Balance in Patients with Limb Dysfunction after Stroke

摘要 目的:探究经皮神经电刺激联合康复机器人对脑卒中后肢体功能障碍患者步态及平衡水平的影响。方法:选取2021年4月—2024年4月苏州广慈肿瘤医院康复科收治的96例脑卒中后肢体功能障碍患者为研究对象,采用随机数表法将其分为对照组和研究组,各48例。对照组采用常规康复训练干预,研究组在对照组基础上采用经皮神经电刺激联合康复机器人干预。两组均持续干预8周。比较两组步态功能、平衡能力和关节主动活动度。结果:干预后,研究组步行速度快于对照组,步长、步幅均长于对照组,步频高于对照组,差异均有统计学意义(P<0.05)。干预后,研究组Berg平衡量表评分高于对照组,差异有统计学意义(P<0.05)。干预后,研究组肩关节外展、肘关节屈曲、腕关节掌屈、髋关节内外旋、膝关节屈伸活动度均大于对照组,差异均有统计学意义(P<0.05)。结论:经皮神经电刺激联合康复机器人干预能有效改善脑卒中后肢体功能障碍患者的步态功能、平衡水平和关节活动度,有利于肢体功能恢复。 Objective:To explore the effects of percutaneous electrical nerve stimulation combined with rehabilitation robot on gait and balance level of patients with limb dysfunction after stroke.Methods:A total of 96 patients with limb dysfunction after stroke admitted to the Department of Rehabilitation in Suzhou Guangci Cancer Hospital from April 2021 to April 2024 were selected as the study objects,and were divided into a control group and a study group by random number table method,with 48 cases in each group.The control group received routine rehabilitation training intervention,and the research group received percutaneous nerve electrical stimulation combined with rehabilitation robot intervention on the basis of the control group.Both groups were treated for 8 weeks.Gait function,balance and active joint motion were compared between the two groups.Results:After intervention,the walking speed of the study group was faster than that of the control group,the stride length and stride length were longer than that of the control group,and the step frequency of the control group was higher,with statistical significance(P<0.05).After intervention,the Berg Balance Scale score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After the intervention,shoulder abduction,elbow flexion,wrist metacarpal flexion,hip joint internal and external rotation,knee flexion and extension motion in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Percutaneous electrical nerve stimulation combined with rehabilitation robot intervention can effectively improve the gait function,balance level and joint motion of patients with limb dysfunction after stroke,which is conducive to the recovery of limb function.
