摘要
类风湿性关节炎(RA)是一种病因不明的自身免疫疾病,除主要的关节病变,其病变范围往往涉及多系统多器官。目前该病的诊断和治疗多聚焦于关节局部病变,而肠道症状在内的全身表现尚未受到关注。“土枢四象,一气周流”理论强调中焦脾胃对四象和全身气机的调控作用。“肠-关节轴”则从器官交互角度,关注到肠道与关节的联系。二者在认识RA的发病机制和治疗思路上有着共通之处。文章拟探讨两种理论的关联性,为RA的临床诊治提供一定帮助。
Rheumatoid arthritis(RA)is an autoimmune disease with unknown etiology,and its pathological changes often involve multiple systems and organs besides the major joint lesions.Currently,the diagnosis and treatment of this disease primarily focus on local joint lesions,while systemic manifestations,including intestinal symptoms,have not received much attention.The theory of"the earth element(spleen and stomach)as the pivot coordinating the other four zang-organs with the circulation of a single qi"emphasizes the regulatory role of the spleen and stomach in the middle energizer on the other four zang-organs and the body's qi mechanism.The"gut-joint axis",from the perspective of organ interaction,focuses on the connection between the gut and the joints.There are commonalities between these two theories in understanding the pathogenesis and treatment strategies of RA.This article aims to explore the relationship between these two theories to provide some assistance in the clinical diagnosis and treatment of RA.
