摘要
目的:分析早中期直肠癌患者行腹腔镜手术治疗的临床疗效。方法:选取2019年1月—2023年3月汉中市城固县医院收治的90例早中期直肠癌患者作为研究对象,以随机数字表法分为对照组(n=45,行开腹手术)和观察组(n=45,行腹腔镜手术)。比较两组手术指标、康复指标、并发症发生情况及炎性指标。结果:观察组切口长度短于对照组,术中出血量少于对照组,手术耗时长于对照组(P<0.05);两组清扫淋巴结数量对比,无统计学差异(P>0.05)。观察组术后首次下床时间、首次肛门排气时间、恢复进食时间及住院时间短于对照组(P<0.001)。观察组并发症发生率低于对照组(P=0.025)。术后24h,两组C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)水平均高于术前,观察组CRP、IL-6、PCT水平低于对照组(P<0.05)。结论:早中期直肠癌患者行腹腔镜手术的效果显著,其手术切口小,出血量少,患者术后恢复快,炎性反应轻,且并发症少。
Objective:To analyze the clinical effect of laparoscopic surgery on patients with early and middle rectal cancer.Meth‐ods:A total of ninety patients with early and middle rectal cancer treated in Hanzhong Chenggu County Hospital from January 2019 to March 2023 were selected as the study objects,and were divided into control group(n=45,laparotomy)and observation group(n=45,laparoscopic surgery)by random number table method.Surgical indexes,rehabilitation indexes,complications and in‐flammatory indexes were compared between the two groups.Results:The incision length of observation group was shorter than that of control group,the amount of intraoperative blood loss was less than that of control group,and the operation time was longer than that of control group(P<0.05);there was no significant difference in the number of lymph nodes between the two groups(P>0.05).The first time to get out of bed,the first time to exhaust the anus,the time to resume eating and the length of hospital stay in the obser‐vation group were shorter than those in the control group(P<0.001).The complication rate of observation group was lower than that of control group(P=0.025).24 h after operation,the levels of C-reactive protein(CRP),interleukin-6(IL-6)and procalcitonin(PCT)in two groups were higher than those before operation,and the levels of CRP,IL-6 and PCT in observation group were lower than those in control group(P<0.05).Conclusion:Laparoscopic surgery for patients with early and middle rectal cancer has significant effects,with small incision,less blood loss,quick postoperative recovery,light inflammatory response,and fewer complications.
