摘要
目的探讨认知行为疗法(CBT)对首发抑郁障碍患者外周血脑源性神经营养因子前体(proBDNF)水平与注意功能的影响。方法纳入2020年1月—2021年12月在云南省精神病医院住院治疗的符合入组标准的首发抑郁障碍患者64例作为研究对象,按照入组先后顺序分为联合组和对照组,每组32例。对照组单服艾司西酞普兰治疗,联合组在服用艾司西酞普兰的基础上,联合CBT疗法。分别测量治疗前后患者外周血proBDNF,并使用24项汉密尔顿抑郁量表(HAMD-24)评估治疗前后患者的抑郁水平,使用持续性操作测验(CPT)评估患者治疗前后的注意功能。采用SPSS分析两组患者治疗前后组内、组间的差异性。结果两组患者在治疗前proBDNF值、HAMD-24评分、CPT评分差异均无统计学意义(P>0.05)。治疗第8周末,联合组proBDNF值、HAMD-24评分均低于对照组,CPT评分(2位数、3位数、4位数)均高于对照组,差异有统计学意义(P<0.05)。结论认知行为疗法能够显著减低患者的抑郁症状,降低proBDNF值,提高注意功能。
Objective To explore the effects of cognitive behavioral therapy(cognitive behavior therapy,CBT)on peripheral proBDNF levels and attentional function in first-episode depressive disorder patients.Methods 64 patients with first-episode depressive disorder who met the enrollment criteria from January 2020 to December 2021 were included as the study subjects.According to the order of enrollment,32 cases were included to combined group at first,and then 32 cases were included to the control group.The control group received escitalopram treatment alone,and the combined group combinewith CBT at the base ofcontrol group.Precursor of brain-derived neurotrophic factor(proBDNF)in peripheral blood were measured before and after treatment.The 24-item Hamilton Depression Scale(HAMD-24)and the continuous performance test(CPT)were used to measure the level of depression and the attentional function before and after treatment.Differences within and between groups before and after treatment were analyzed by SPSS.Results There was no difference in pre-treatment proBDNF value,HAMD-24 score and CPT score in the 2 groups.After 8 weeks,the proBDNF value and HAMD-24 scores in the combination group were lower than the control group,and the CPT scores(2,3 and 4 digits)were higher than the control group,and were statistically different(P<0.05).Conclusion Cognitive behavioral therapy can significantly reduce depressive symptoms,reduce proBDNF,and improve attentional function.
