摘要
目的探讨日间手术模式下射频消融(RFA)闭合术治疗下肢静脉曲张的临床疗效及安全性。方法收集2021年12月至2022年5月新疆维吾尔自治区人民医院收治的日间手术模式下治疗的101例大隐静脉曲张患者(112条肢体)的临床资料,按照手术方法的不同将患者分为高位结扎组(n=50)和RFA组(n=51)。比较两组患者围手术期相关指标、围手术期并发症发生率及术后3个月大隐静脉主干闭塞率。结果RFA组患者的术前禁食、禁水时间,手术时间,术后禁食、禁水时间,术后下床时间均短于高位结扎组患者,术中出血量少于高位结扎组患者,术中VAS评分高于高位结扎组患者,术后6 h VAS评分低于高位结扎组患者,差异均有统计学意义(P<0.05)。术后3个月,RFA组患者的大隐静脉主干闭合率为98.2%(56/57),高位结扎组患者的大隐静脉主干闭合率为100%(55/55)。两组患者围手术期并发症的总发生率比较,差异无统计学意义(P>0.05)。结论RFA在日间手术模式下治疗下肢静脉曲张安全、有效,能够缩短术前禁食、禁水时间,手术时间,术后禁食、禁水时间,术后下床时间,减少术中出血量和术后疼痛程度,且不会增加围手术期相关并发症。
Objective To investigate the clinical efficacy and safety of radiofrequency ablation(RFA)closure surgery in the treatment of varicose vein of lower extremity under daytime operation mode.Method The clinical data of 101 patients(112 extremities)with great saphenous varicose veins treated under daytime operation mode in the People's Hospital of Xinjiang Uygur Autonomous Region from December 2021 to May 2022 were collected.According to the different operational methods,the patients were divided into high ligation group(n=50)and RFA group(n=51).The perioperative related indicators,incidence of perioperative complications,and the occlusion rate of the trunk of great saphenous vein at 3 months after surgery between the two groups of patients were compared.Result The preoperative fasting and water time,operational time,postoperative fasting and water time,postoperative getting out of bed time in the RFA group were shorter than those in the high ligation group,intraoperative blood loss was less than that in the high ligation group,intraoperative VAS score was higher than that in the high ligation group,postoperative 6 h VAS score in the RFA group was lower than that in the high ligation group,all the differences above were statistically significant(P<0.05).Three months after surgery,the closure rate of the trunk of great saphenous vein in the RFA group was 98.2%(56/57),while the closure rate of the trunk of great saphenous vein in the high ligation group was 100%(55/55).There was no significant difference in the incidence of perioperative complications between the two groups(P>0.05).Conclusion RFA is a minimally safe and effective method for the treatment of varicose veins of lower extremities in daytime operation mode,which can shorten the preoperative fasting and water time,operational time,postoperative fasting and water time,and postoperative getting out of bed time,reduce intraoperative blood loss and postoperative pain,and do not increase perioperative related complications.
