摘要
目的 研究宫颈扩张球囊与催产素联合人工破膜在孕晚期分娩催产中的应用价值。方法 方便选取2022年1—12月河源市妇幼保健院收治的237例孕晚期分娩产妇为研究对象,运用随机数表法分为对照组(132例,宫颈扩张球囊+催产素催产)和观察组(105例,宫颈扩张球囊+人工破膜+催产素催产)。比较各组临产时间、放置球囊前后宫颈Bishop评分、阴道分娩率、并发症发生率、新生儿阿普加(Apgar)评分。结果 放置球囊前,两组宫颈Bishop评分对比,差异无统计学意义(P>0.05);放置球囊后,观察组宫颈Bishop评分高于对照组,且观察组临产时间短于对照组,差异有统计学意义(P均<0.05)。观察组阴道分娩率为94.29%,高于对照组的81.06%,差异有统计学意义(P<0.05)。观察组并发症发生率为2.86%,低于对照组的9.85%,差异有统计学意义(P<0.05)。观察组Apgar评分高于对照组,差异有统计学意义(P<0.05)。结论 宫颈扩张球囊与催产素联合人工破膜可显著加快孕晚期分娩产妇的临产时间,促进宫颈成熟、阴道分娩,减少新生儿窒息情况,总体催产效果良好。
Objective To study the application value of cervical dilatation balloon and oxytocin combined with artificial rupture of membranes in labor hypnosis in late pregnancy.Methods A total of 237 pregnant women who gave birth in the third trimester of pregnancy in Heyuan Maternal and Child Health Hospital from January to December 2022 were conveniently selected as the research objects.They were divided into control group(132 cases,cervical dilatation balloon + oxytocin) and observation group(105 cases,cervical dilatation balloon + artificial rupture of membrane + oxytocin) by random number table method.The labor time,cervical Bishop score before and after balloon placement,vaginal delivery rate,complication rate and neonatal Apgar score were compared among the groups.Results Before placing the balloon,there was no statistically significant difference in the Bishop scores between the two groups of cervix(P>0.05);after the placement of the balloon,the Bishop score of the observation group was higher than that of the control group,and the delivery time of the observation group was shorter than that of the control group,and the differences were statistically significant(both P<0.05).The vaginal delivery rate in the observation group was 94.29% higher than that in the control group,which was 81.06%,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 2.86%,which was lower than the control group's 9.85%,and the difference was statistically significant(P<0.05).The Apgar score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Cervical dilatation balloon and oxytocin combined with artificial rupture of membranes can significantly accelerate the time of labor in late pregnancy,promote cervical ripening,vaginal delivery,reduce neonatal asphyxia,and the overall effect of labor hypnosis is good.
