加味桃红四物汤对骨质疏松性腰椎压缩性骨折瘀血阻滞型患者腰椎功能的影响 被引量:2

Effect of Modified Taohong Siwu Decoction on Lumbar Function in Osteoporotic Vertebral Compression Fractures of Blood Stasis Obstruction Type

摘要 目的:观察加味桃红四物汤对骨质疏松性腰椎压缩性骨折瘀血阻滞型患者腰椎功能的影响。方法:选取114例骨质疏松性腰椎压缩性骨折瘀血阻滞型患者,按随机数字表法分为对照组及观察组各57例。对照组采用经皮椎体后凸成形术治疗,观察组在对照组基础上联合桃红四物汤口服治疗。比较2组临床疗效,比较2组治疗前后血清前列腺素E2(PGE2)、白细胞介素-17(IL-17)、环氧合酶-2(COX-2)、骨碱性磷酸酶(ALP)、护骨素(OPG)、健康状况调查简表评分(SF-36)、疼痛视觉模拟评分法(VAS)评分、腰椎功能障碍指数(ODI)的变化。结果:治疗后,2组血清PGE2、COX-2、IL-17水平均较治疗前下降(P<0.05),观察组血清PGE2、COX-2、IL-17水平均低于对照组(P<0.05)。治疗后,2组血清ALP、OPG水平均较治疗前升高(P<0.05),观察组ALP、OPG水平均高于对照组(P<0.05)。治疗后,2组VAS评分、ODI评分均较治疗前下降,SF-36评分均较治疗前上升(P<0.05);观察组VAS评分、ODI评分均低于对照组,SF-36评分高于对照组(P<0.05)。观察组骨折愈合时间、疼痛缓解时间均短于对照组,差异有统计学意义(P<0.05)。观察组总有效率为96.49%,对照组总有效率为80.70%,2组比较,差异有统计学意义(P<0.05)。结论:加味桃红四物汤治疗骨质疏松性腰椎压缩性骨折瘀血阻滞型患者,有助于减轻炎症反应,调节骨代谢指标值,促进腰椎功能恢复,提高临床疗效。 Objective:To observe the effect of Modified Taohong Siwu Decoction on lumbar function in osteoporotic vertebral compression fractures of blood stasis obstruction type.Methods:A total of 114 cases of patients with osteoporotic vertebral compression fractures of blood stasis obstruction type were divided into the control group and the observation group according to the random number table method,with 57 cases in each group.The control group was treated with percutaneous kyphoplasty,and the observation group was additionally treated with Modified Taohong Siwu Decoction orally based on the treatment of the control group.Clinical effects were compared between the two groups.The changes in serum prostaglandin E2(PGE2),interleukin-17(IL-17),cyclooxygenase-2(COX-2),bone alkaline phosphatase(ALP),osteoprotegerin(OPG),the scores of 36-Item Short Form Health Survey(SF-36),Visual Analogue Score(VAS)of pain,and Oswestry Disability Index(ODI)were compared between the two groups before and after treatment.Results:After treatment,the levels of PGE2,COX-2,and IL-17 in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of ALP and OPG in serum in the two groups were increased when compared with those before treatment(P<0.05),and the above two levels in the observation group were higher than those in the control group(P<0.05).After treatment,the scores of VAS and ODI in the two groups were decreased when compared with those before treatment,and SF-36 scores were increased when compared with those before treatment(P<0.05);the scores of VAS and ODI in the observation group were lower than those in the control group,and SF-36 in the observation group was higher than that in the control group(P<0.05).The fracture healing time and the pain remission time in the observation group were shorter than those in the control group,the difference being significant(P<0.05).The total effective rate was 96.49%in the observation group and 80.70%in the control group,the difference being significant(P<0.05).Conclusion:For patients with lumbar function in osteoporotic vertebral compression fractures blood stasis obstruction type,Modified Taohong Siwu Decoction can help relieve inflammatory reactions,regulate bone metabolism indexes,promote the recovery of lumbar function,and enhance clinical effects.
