心肌标志物联合检测在急性心肌梗死诊断中的应用价值分析

Analysis of the application value of combined detection of myocardialmarkers in the diagnosis of acute myocardial infarction

摘要 目的探讨急性心肌梗死诊断中联合检测心肌标志物的应用价值。方法选择2021年8月-2023年8月收治的300例急性心肌梗死患者,设为观察组,另选择同期入院诊断的非急性心肌梗死心脏神经官能症患者300例,设为对照组。检测两组患者肌钙蛋白I(cTnI)、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、脑利钠肽(BNP)水平。结果观察组cTnI、MYO、CK-MB、BNP水平均高于对照组(P<0.05);四项指标联合检测的敏感性、特异性均高于任意一项指标单独检测。结论cTnI、MYO、CK-MB、BNP等心肌标志物能反映心肌细胞损伤程度,联合检测的敏感性和特异性高于各指标单项检测,能提高早期AMI诊断的准确性,有利于为临床救治和预后判断提供有效参考,降低患者死亡率,有实用价值。 Objective To investigate the value of combined detection of cardiac markers in the diagnosis of acute myocardial infarction.Methods A total of 300 patients with acute myocardial infarction who were admitted to the hospital from August 2021 to August 2023 were selected as the observation group,and 300 patients with cardiac neurosis without acute myocardial infarction who were admitted during the same period were selected as the control group.The levels of cardiac troponinⅠ(cTnI),myoglobin(MYO),creatine kinase isoenzyme(CK-MB)and brain natriuretic peptide(BNP)were detected in the two groups.Results The levels of cTnl,MYO,CK-MB,and BNP in the observation group were higher than those in the control group(P<0.05).the sensitivity and specificity of combined detection of the four indicators were higher than those of any indicator alone.Conclusion cTnl,MYO,CK-MB,BNP and other myocardial markers can reflect the degree of myocardial cell injury.The sensitivity and specificity of combined detection are higher than those of each index single detection,which can improve the accuracy of early diagnosis of AMI,provide effective reference for clinical treatment and prognosis judgment,reduce the mortality of patients,and have practical value.
