摘要
目的探讨动态心电图诊断无症状性心肌缺血的临床价值研究。方法将2022年1—12月在本院治疗的92例疑似无症状性心肌缺血患者作为研究对象,均行常规心电图及动态心电图检查,将常规心电图作为对照组,将动态心电图检查作为观察组,以冠脉造影结果作为金标准,对比两组的诊断效能、心电图指标。结果金标准检查结果显示,两组对比差异不明显,无统计学意义(P>0.05);观察组对无症状性心肌缺血诊断的敏感度、特异度、准确度、阳性预测值、阴性预测值均高于对照组(P<0.05);观察组ST段下移次数、ST段下移幅度、ST段下移维持时间、Ⅳ、心率均大于对照组(P<0.05)。结论动态心电图诊断无症状性心肌缺血的临床价值显著,能提高诊断效能,增加诊断的敏感度及特异度,更严谨地反映心电活动状态,利于临床早期诊治。
Objective To explore clinical value of dynamic electrocardiogram in diagnose of asymptomatic myocardial ischemia.Methods The paper chose 92 suspected asymptomatic myocardial ischemia patients in our hospital from January 2022 to December 2022 as study subjects,control group was treated with routine electrocardiogram,and observation group observation group with dynamic electrocardiogram examinations.Diagnostic effect and electrocardiogram indicators between two groups were compared with gold standard of coronary angiography result.Results Gold standard examination showed,there was no significant difference between two groups,with no statistical significance(P>0.05).Sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of observation group for diagnosis of asymptomatic myocardial ischemia were higher than control group(P<0.05).Number and amplitude of ST segment descent,duration of ST segment descent,Ⅳ,and heart rate in observation group was higher than control group(P<0.05).Conclusion Dynamic electrocardiogram can achieve significant clinical value in diagnose of asymptomatic myocardial ischemia,can improve diagnostic efficiency,increase diagnostic sensitivity and specificity,and reflect state of cardiac activity more rigorously,which is conducive to early clinical diagnosis and treatment.
