摘要
目的观察布地奈德+阿奇霉素+特布他林雾化吸入治疗支原体肺炎患儿的效果及对炎症反应、肺功能及T淋巴细胞亚群的影响。方法选择2021年1月至12月收治的84例支原体肺炎患儿作为研究对象,随机将其分为对照组和观察组,各42例。对照组给予阿奇霉素+特布他林治疗,观察组在对照组基础上给予布地奈德治疗。比较两组的治疗效果。结果观察组的咳嗽、体温升高、肺部啰音、肺部病灶消失时间及支原体-IgM抗体转阴时间均短于对照组(P<0.05)。治疗后,观察组的降钙素原(PCT)、粒细胞集落刺激因子(G-CSF)、白细胞介素-6(IL-6)、铁蛋白水平均低于对照组(P<0.05)。治疗后,观察组的用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、用力呼出50%肺活量时的平均呼气流速(MEF50)、最大呼气流速峰值(PEF)均高于对照组(P<0.05)。治疗后,观察组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于对照组(P<0.05)。结论布地奈德+阿奇霉素+特布他林雾化吸入治疗支原体肺炎患儿的效果显著,可显著改善患儿症状、体征,降低炎症因子水平,提高肺功能及免疫功能。
Objective To observe the effect of budesonide+azithromycin+terbutaline aerosol inhalation in the treatment of children with mycoplasma pneumonia and its influences on inflammatory response,lung function and T lymphocyte subsets.Methods A total of 84 children with mycoplasma pneumonia admitted from January to December 2021 were selected as the research objects,and randomly divided into control group and observation group,with 42 cases in each group.The control group was treated with azithromycin+terbutaline,and the observation group was treated with budesonide on the basis of the control group.The therapeutic effects of the two groups were compared.Results The disappearance time of cough,elevated body temperature,lung rales,lung lesions and negative conversion time of mycoplasma-IgM antibody in the observation group were shorter than those in the control group(P<0.05).After treatment,the levels of procalcitonin(PCT),granulocyte colony-stimulating factor(G-CSF),interleukin-6(IL-6)and ferritin in the observation group were lower than those in the control group(P<0.05).After treatment,the forced vital capacity(FVC),forced expiratory volume in one second(FEV1),maximal expiratory flow at 50% of the vital capacity(MEF50)and peak expiratory flow(PEF)in the observation group were higher than those in the control group(P<0.05).After treatment,the CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05).Conclusion Budesonide+azithromycin+terbutaline aerosol inhalation has a significant effect in the treatment of children with mycoplasma pneumonia,which can significantly improve the symptoms and signs of children,reduce the levels of inflammatory factors,and improve lung function and immune function.
