RBRVS与DRGs相结合的绩效方案在公立医院临床活动中的应用价值

摘要 目的 探讨资源为基础的相对价值比率(Resource based relative value scale, RBRVS)和疾病诊断相关分组(Diagnosis related group, DRGs)相结合的绩效管理方案在公立医院临床活动中的应用价值。方法 选取我院内分泌科及骨科患者为研究对象,2022年1月至3月两科室绩效分配采用RBRVS为基础的绩效方案,2023年1月至3月采用RBRVS与DRGs相结合的绩效方案,获得并比较两科室不同年份同月份实施两种不同绩效方案的床位周转率、住院时间、平均费用、死亡率和主观满意度。结果 与RBRVS为基础的绩效方案相比,实施RBRVS与DRGs相结合的绩效方案后,两科室床位周转率、住院时间、平均费用、病死率均显著降低(P<0.05),主观满意度提高(P<0.05)。结论 RBRVS与DRGs相结合的绩效方案有利于减少临床住院时间、费用和病死率,提高床位利用率和主观满意度,值得临床推广。 Objective To explore the application value of a performance management plan that combines Resource based relative value scale(RBRVS)and Diagnosis related group(DRGs)in public hospital clinical activities.Methods Patients from the endocrinology and orthopedic departments of our hospital were selected as the research subjects.From January to March 2022,the performance allocation of the two departments was based on RBRVS performance plan.From January to March 2023,a performance plan combining RBRVS and DRGs was adopted for the two departments.Obtain and compare the bed turnover rate,hospitalization time,average cost,mortality rate,and subjective satisfaction of the two departments implementing two different performance plans in different years and months.Results Compared with the RBRVS based performance plan,the implementation of the RBRVS combined with DRGs performance plan significantly reduced the bed turnover rate,hospitalization time,average cost,and mortality rate in both departments(P<0.05),and improved subjective satisfaction(P<0.05).Conclusion The performance plan combining RBRVS and DRGs is beneficial to reducing clinical hospitalization time,costs,and mortality,improving bed utilization and subjective satisfaction,and is worthy of clinical promotion.
