摘要
目的系统评价中药灌肠疗法对慢性阻塞性肺疾病急性加重期(Acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者血气分析、肺功能的疗效及安全性。方法计算机检索PubMed、The Cochrane library、Wanfang、CBM、CNKI和VIP数据库,检索时限为建库起至2021年12月12日。由两名评价者提取数据、独立评价并交叉核对纳入文献质量,采用Review Manager 5.4软件对符合条件的文献进行Meta分析。使用相对风险(RR)、平均差(MD)以及95%置信区间(95%CI)分析主要结果。根据Cochrane手册偏倚风险工具评估相关研究的质量。结果共纳入了25项随机对照试验。与单纯常规治疗比较,中药灌肠联合常规治疗可显著降低动脉二氧化碳分压(PaCO_(2))[MD=-4.27,95%CI(-5.82,-2.71),P<0.0001];提高肺功能FEV1/FVC水平[MD=6.35,95%CI(3.47,9.22),P<0.0001]等。结论中药灌肠可改善AECOPD患者的血气分析和肺功能结果。然而,具体的药物使用、治疗频率和剂量需要更高质量、更大样本的随机对照试验证实。
Objective To evaluate the clinical efficacy and safety of traditional Chinese medicine(TCM)enema therapy on blood gas analysis and pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods PubMed,The Cochrane Library,Wanfang Data,CBM,CNKI,and VIP databases were searched from their inception to December 12,2021.Two investigators independently extracted data,assessed study quality,and conducted cross-verification of included trials.Review Manager 5.4 was employed for Meta-analysis of eligible trials.The main outcomes were analyzed using relative risk(RR),mean difference(MD),and 95% confidence interval(95% CI).The quality of relevant trials was assessed using Cochrane Handbook for Systematic Reviews of Interventions.Results A total of 25 randomized controlled trials(RCTs)were included.Compared with conventional treatment alone,TCM enema combined with conventional treatment significantly reduced arterial carbon dioxide partial pressure(PaCO_(2))[MD=-4.27,95% CI(-5.82,-2.71),P<0.0001],and increased the FEV1/FVC ratio[MD=6.35,95% CI(3.47,9.22),P<0.0001].Conclusion TCM enema therapy can improve blood gas analysis and pulmonary function in patients with AECOPD.However,specific drug usage,treatment frequency,and dosage require validation through RCTs with higher quality and larger sample sizes.
