大量输血对急性创伤患者凝血功能的影响

Influence of Massive Blood Transfusion on Blood Coagulation Function in Patients with Acute Trauma

摘要 目的:探讨大量输血对急性创伤患者凝血功能的影响。方法:选取2018年1月—2021年1月南京市高淳人民医院收治的急性创伤患者86例作为研究对象,以随机数字表法分为对照组和观察组,各43例。对照组给予少量输血,观察组给予大量输血。比较两组凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D)及纤维蛋白降解产物(FDP)水平。结果:输血前及输血后6 d,两组PT、TT、APTT、FIB水平比较,差异无统计学意义(P>0.05);输血后1 d,观察组PT、TT、APTT均高于对照组,FIB低于对照组,差异有统计学意义(P<0.05)。输血前及输血后6 d,两组D-D及FDP水平比较,差异无统计学意义(P>0.05);输血后1 d,观察组DD及FDP水平均高于对照组,差异有统计学意义(P<0.001)。结论:急性创伤患者大量输血后,凝血四项、D-D及FDP水平会出现一定变化,致使凝血功能紊乱,治疗时应密切监测患者凝血状态,避免发生并发症。 Objective:To investigate the influence of massive blood transfusions on blood coagulation function in patients withacute trauma.Methods:A total of 86 acute trauma patients admitted to Nanjing Gaochun People's Hospital from January 2018 toJanuary 2021 were selected as the study subjects,and they were divided into control group and observation group by randomnumber table method,with 43 cases in each group.The control group was given a small amount of blood transfusion,and theobservation group was given massive blood transfusion.The levels of prothrombin time(PT),thrombin time(TT),activated partialthromboplastin time(APTT),fibrinogen(FIB),D-dimer(D-D) and fibrin degradation product(FDP) were compared between thetwo groups.Results:There was no statistically significant difference in the levels of PT,TT,APTT and FIB between the two groupsbefore transfusion and on 6 d after transfusion(P>0.05).On 1 d after transfusion,the PT,TT and APTT in the observation groupwere higher than those in the control group,and FIB in the observation group was lower than that in the control group,and thedifference was statistically significant(P<0.05).There were no significant differences in D-D and FDP levels between the twogroups before transfusion and on 6 d after transfusion(P>0.05).On 1 d after transfusion,the D-D and FDP levels in theobservation group were higher than those in the control group,and the differences were statistically significant(P<0.001).Conclusion:After massive blood transfusions in patients with acute trauma,the levels of four items of coagulation,D-D and FDPchange,resulting in coagulation dysfunction,and the coagulation status of patients should be closely monitored during treatment toavoid complications.
