摘要
目的:观察微螺钉种植体支抗用于双颌前突正畸治疗的效果。方法:选取2019年2月至2021年2月该院收治的60例双颌前突患者进行前瞻性研究,按照抽签法将其分为观察组与对照组各30例。对照组给予正畸治疗,观察组在对照组基础上采用微螺钉种植体支抗治疗,比较两组上颌骨相对颅部前后位置关系(SNA角)、上中切牙长轴与前颅底平面交角(U1-SN角)、下颌骨相对颅部前后位置关系(SNB角)、口腔健康相关生活质量(OHRQOL)评分、牙周指标[改良牙菌斑指数(mPLI)、改良龈沟出血指数(mSBI)及探诊深度(PD)]水平和并发症发生率。结果:治疗后,两组SNA角、U1-SN角和SNB角均小于治疗前,且观察组小于对照组,差异有统计学意义(P<0.05);治疗后,两组OHRQOL各维度评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗前后,两组mPLI、mSBI和PD水平比较,差异均无统计学意义(P>0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:微螺钉种植体支抗用于双颌前突正畸治疗可缩小SNA角、U1-SN角和SNB角,提高OHRQOL评分,效果优于单纯正畸治疗。
Objective:To observe effects of micro screw implant anchorage for orthodontic treatment of bimaxillary protrusion.Methods:A prospective study was conducted on 60 patients with bimaxillary protrusion admitted to the hospital from February 2019 to February 2021.They were divided into observation group and control group according to the lottery method,30 cases in each.The control group was given orthodontic treatment,while the observation group was treated with micro screw implant anchorage on the basis of that of the control group.The anteroposterior position of the maxilla relative to the skull(SNA angle),the angle between the long axis of the upper central incisor and the plane of the anterior skull base(U1-SN angle),the anteroposterior position of the mandible relative to the skull(SNB angle),the oral health-related quality of life scale(OHRQOL)score,the periodontal index levels[modified plaque index(mPLI),modified sulcus bleeding index(mSBI)and probing depth(PD)],and the incidence of complications were compared between the two groups.Results:After the treatment,the SNA angles,U1-SN angles and SNB angles of the two groups were smaller than those before the treatment,those in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the scores of each dimension of OHRQOL in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Before and after the treatment,there were no significant differences in the levels of mPLI,mSBI and PD between the two groups(P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Micro screw implant anchorage for orthodontic treatment of bimaxillary protrusion can reduce the SNA angle,the U1-SN angle and the SNB angle,and improve the OHRQOL scores.Moreover,it is superior to single orthodontic treatment.
