摘要
对胸腺瘤在调强放疗中共面野计划与非共面野计划的剂量学进行比较研究,从而得出对患者受益更好的放疗计划。纳入2021年1月—2022年12月蚌埠医学院第一附属医院放疗科收治的80例胸腺瘤不完全切除术后患者,对这80例患者采用相同的计划系统进行共面野和非共面野调强计划设计,其中共面野计划采用机架0°、320°、40°,非共面野计划采用机架0°、330°(床90°)、30°(床90°),将两者得出的计划进行比较,主要比较靶区的适形度、均匀性,危及器官脊髓、心脏、肺的受量,以及对两种计划设计的机器执行效率进行比较。非共面野与共面野相比结果:靶区适形度较差,V20、V30相差不大,心脏平均剂量高269.73cGy,脊髓平均剂量高516.06cGy。研究发现,非共面野计划设计脊髓、心脏的受量稍高,但肺的低剂量受量明显减少,可以有效预防患者放疗晚期放射性肺炎的发生。
The purpose of studying thymoma is to compare dosiology in the Coplanar field plan and Noncoplanar filed plan in the intensity modulated radiation therapy,which is in order to receive a better Radiation therapy plan for the benefit of the patients.From the January of 2020 to the January of 2022,our department had treated 30 patients who had received Incomplete thymoma resection,and we used the same planning system to conduct designing Coplanar field and Noncoplanar filed Intensitymodulated plan,in which Coplanar field plan used rack 0 degree,320 degree,40 degree and Noncoplanar filed plan used rack 0 degree,330 degree(bed 90 degree),30 degree(bed 90 degree),then we compared the two plans,mainly comparing the Conformal degree,homogeneity of target section and(vital organ at risk)spinal cord,heart,lung dose and the effection of vulnerable machine designed by the two plans.Comparing Noncoplanar filed and coplanar filed:the Conformal degree of target section is worse than coplanar filed,the V5 of two lungs decreased 11.65%on averge,the difference between V20 and V30 is very little,the averge dose of heart is 269.73cGy higher then Coplanar filed,the averge dose of spinal cord is 516.06cGy higher.Using Noncoplanar filed plan to design the spinal cord and heart dose is little higher,but the low dose of lung is decreased apparently,which is able to efficiently prevent the occurrence of radiation pneumonitis for those patients who are in the late stage after radiotherapy.
