摘要
目的比较光子剂量算法(AXB)与各向异性分析算法(AAA)在非小细胞肺癌(NSCLC)放射治疗中的剂量学差异。方法选取北部战区总医院自2020年6月至2021年6月收治的20例NSCLC患者为研究对象。根据患病部位将其分为左肺组与右肺组,每组各10例。两组患者均分别采用AXB、AAA两种算法计划。记录并比较两种算法的靶区以及危及器官的剂量学差异。结果左肺组患者中,AAA算法的靶区最大剂量、患侧肺受到5 Gy剂量照射的体积百分比、脊髓平均剂量均优于AXB算法,AXB算法的食管平均剂量优于AAA算法,差异均有统计学意义(P<0.05)。右肺组患者中,AAA算法的靶区最大剂量优于AXB算法,AXB算法的食管受到30 Gy剂量照射的体积百分比、食管平均剂量均优于AAA算法,差异均有统计学意义(P<0.05)。结论NSCLC放射治疗中,左肺病变优先选择AAA算法,右肺病变优先选择AXB算法。
Objective To compare the dosimetry differences between acuros external beam algorithm(AXB)and anisotropic analytical algorithm(AAA)in radiotherapy of non-small cell lung cancer(NSCLC).Methods Twenty NSCLC patients admitted to General Hospital of Northern Theater Command from June 2020 to June 2021 were selected as the research subjects.They were divided into left lung group and right lung group according to the location of disease,with 10 cases in each group.AXB and AAA algorithms were used in both groups,respectively.The dosimetry differences of target areas and organs at risk between the two algorithms were recorded and compared.Results In the left lung group,the dosemax(D;)of target region,volume 5(V;)of affected lung and D;of spinal cord of AAA algorithm were better than that of AXB algorithm,and the dosemean(D;)of esophagus of AXB algorithm was better than that of AAA algorithm,with statistical significance(P<0.05).In the right lung group,the target D;of AAA algorithm was better than that of AXB algorithm,and the esophageal volum 30(V;)and esophageal D;of AXB algorithm were better than those of AAA algorithm,with statistical significance(P<0.05).Conclusion In NSCLC radiotherapy,AAA algorithm is preferred for left lung lesions and AXB algorithm for right lung lesions.
