摘要
目的探讨彩色多普勒超声血流显像(CDFI)参数在子宫肌瘤(UF)与子宫肉瘤(US)鉴别诊断中的应用价值。方法选择58例US患者与65例UF患者,均行CDFI检查。比较US与UF患者的声像特征及血流信号,以手术病理诊断结果作为金标准,计算声像特征、血流信号单项及联合检查在US中的鉴别诊断效能。结果US患者中瘤体边界清晰、内部结构实性、组织低回声占比低于UF患者,差异有统计学意义(P<0.05);US患者中血流信号稀疏占比低于UF患者,血流信号丰富占比高于UF患者,差异有统计学意义(P<0.05);声像特征+血流信号联合鉴别诊断US结果与病理结果一致性(Kappa=0.773)优于声像特征(Kappa=0.590)、血流信号(Kappa=0.624)单项;声像特征+血流信号联合鉴别诊断US敏感度高于声像特征与血流信号单项,声像特征+血流信号联合鉴别诊断US准确率高于声像特征,差异有统计学意义(P<0.05)。结论US与UF在CDFI声像特征及血流信号方面具有差异,分析CDFI的声像特征及血流信号可为鉴别诊断US与UF提供准确信息,进而为US与UF患者制定更为合理的治疗方法。
Objective To explore the application value of color Doppler flow imaging(CDFI)parameters in the differential diagnosis of uterine fibroid(UF)and uterine sarcoma(US).Methods 58 US patients and 65 UF patients were selected for CDFI examination.The ultrasonographic features and blood flow signals of US and UF patients were compared,and the surgical pathological diagnosis results were used as the gold standard to calculate the differential diagnosis efficiency of ultrasonographic features and blood flow signals by single and combined examinations in US.Results The proportion of clear tumor boundary,solid internal structure and hypoechoic tissue in US patients was lower than that in UF patients,the difference was statistically significant(P<0.05).The proportion of sparse blood flow signals in US patients was lower than that in UF patients,and the proportion of rich blood flow signals was higher than that in UF patients,the difference was statistically significant(P<0.05).The consistency between the combination of ultrasonographic features and blood flow signals in the differential diagnosis of US results(Kappa=0.773)was better than that of ultrasonographic features(Kappa=0.590)and blood flow signals(Kappa=0.624).The sensitivity of combination of ultrasonographic features and blood flow signals in the differential diagnosis of US was higher than that of the ultrasonographic features and blood flow signals,and the accuracy of combination of ultrasonographic features and blood flow signals was higher than that of ultrasonographic features,the difference was statistically significant(P<0.05).Conclusion There are differences between US and UF in CDFI ultrasonographic features and blood flow signals.Analysis of CDFI features features and blood flow signal can provide accurate information for differential diagnosis of US and UF,and then formulate more reasonable treatment for US and UF patients.
