摘要
目的探讨腹腔镜子宫肌瘤剔除术对子宫肌瘤患者内分泌及性功能的影响。方法选取2017年1月~2018年6月我院收治的子宫肌瘤子宫增大>10周孕龄需行子宫肌瘤剥除术80例患者,按照随机数字表法分为对照组和观察组,每组各40例。对照组实施常规开腹手术,观察组实施腹腔镜下子宫肌瘤剥除术,比较两组术后1年自然妊娠与子宫肌瘤复发情况、性功能和性激素水平变化情况。结果观察组术后1年自然妊娠率高于对照组(P<0.05);术后1年,观察组性功能指标女性性生活质量标准量表(FSFI)各分项得分及总分高于对照组(P<0.05),术后1年月经第2天基础激素水平,观察组血清雌二醇(E2)水平高于对照组,卵泡刺激素(FSH)水平低于对照组,差异均有统计学意义(P<0.05)。结论对子宫肌瘤患者行腹腔镜子宫肌瘤剔除术,能有效促进术后妊娠,改善患者性功能,促进性激素水平恢复。
Objective To investigate the effect of laparoscopic myomectomy on endocrine and sexual function in patients with uterine fibroids. Methods A total of 80 patients with uterine enlargement over 10 gestational weeks who were needed hystero myomectomy from January 2017 to June 2018 were selected and divided into control group and observation group according to random number table method, 40 cases in each group. Conventional laparotomy was performed in the control group, and laparoscopic hysteromyomectomy was performed in the observation group. Then the recurrence of hysteromyoma and natural pregnancy rate, sexual function and sex hormone levels in the two groups one year after operation were compared. Results The natural pregnancy rate 1 year after operation was higher than that in the control group(P<0.05). 1 year after operation, the sub-item scores and total scores of female sexual quality of life standard scale(FSFI) in the observation group were higher than those in the control group(P<0.05). On the second day of menstruation one year after operation, the level of serum estradiol(E2) in the observation group was higher than control group, and the level of follicle stimulating hormone(FSH) was lower than control group, the differences were statistically significant(P<0.05). Conclusion Laparoscopic myomectomy for hysteromyoma can effectively promote postoperative pregnancy, improve sexual function and promote the recovery of sex hormone levels.
