摘要
基于运动的心脏康复(cardiac rehabilitation,CR)是国际公认的IA类推荐,适用于特定心血管疾病(cardiovascular diseases,CVD)和射血分数降低的心力衰竭(heart failure,HF)患者。尽管中等强度连续训练(moderate-intensity continuous training,MICT)一直是心脏康复领域采用的主要运动方式,但越来越多的证据表明,高强度间歇训练(high-intensity interval training,HIIT)似乎比MICT在改善心脏病患者的康复效果方面更有效。现综述HIIT运动训练方案对心脏病患者峰值摄氧量(peak oxygen uptake,VO2peak)的影响及对CVD和HF患者肺、心、血管和骨骼肌系统的有益生理机制。并与MICT相比较,展现HIIT对心血管疾病死亡率、依从性、安全性等方面的优越性;最后提出了HIIT运动强度的研究进展。
Exercise-based cardiac rehabilitation(CR)is an internationally recognized class IA recommendation for patients with specific cardiovascular diseases(CVD)and heart failure(HF)patients with reduced ejection fraction.Although moderate-intensity continuous training(MICT)has been the dominant exercise modality used in the cardiac rehabilitation field,there is a growing evidence that high-intensity interval training(HIIT)appears to be more effective in improving the recovery of HF patients than MICT.This review summarized the effect of HIIT on peak oxygen uptake(VO2peak)in HF patients and the mechanisms underlying physiological benefits to the lung,heart,vascular and skeletal muscle systems in patients with CVD and HF.We also described the superiority of HIIT on cardiovascular disease mortality,compliance,and safety compared with those of MICT.Finally,the research progress of HIIT exercise intensity is proposed.
