摘要
目的:分析红细胞参数在贫血患者中的诊断价值。方法:回顾性分析2019年4月至2022年4月该院收治的82例贫血患者的临床资料,根据贫血类型将其分为地中海贫血42例,设为地中海贫血组;缺铁性贫血40例,设为缺铁性贫血组,另选取同期40名健康体检者,设为对照组,检测三组红细胞参数[红细胞计数(RBC)、红细胞分布宽度(RDW)、血红蛋白(Hb)、红细胞平均体积(MCV)]水平,并绘制受试者工作特征(ROC)曲线分析红细胞参数在贫血患者中的诊断价值。结果:缺铁性贫血组、地中海贫血组RBC、Hb水平均低于对照组,且缺铁性贫血组低于地中海贫血组,缺铁性贫血组、地中海贫血组RDW、MCV水平均高于对照组,且缺铁性贫血组高于地中海贫血组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,红细胞参数RBC、RDW、Hb、MCV单项及联合检测诊断贫血的AUC分别为0.840、0.766、0.816、0.690、0.938,其中RBC、RDW、Hb单项及联合检测的AUC均>0.7,且联合检测诊断贫血的AUC最高,MCV的AUC<0.7,在贫血诊断中的效能较差。结论:RBC、RDW、Hb单项及联合检测在贫血诊断中均具有较高的效能,且联合检测诊断效能最高,MCV在贫血诊断中效能较差。
Objective:To analyze diagnostic value of red blood cell parameters in patients with anemia.Methods:The clinical data of 82 patients with anemia admitted to the hospital from April 2019 to April 2022 were retrospectively analyzed.According to the classification of anemia,they were divided into 42 cases of Mediterranean anemia(Mediterranean anemia group)and 40 cases of iron deficiency anemia(iron deficiency anemia group).Another 40 healthy subjects were selected as the control group.The red blood cell parameter levels[red blood cell count(RBC),red blood cell distribution width(RDW),hemoglobin(Hb),mean corpuscular volume(MCV)]of the three groups were detected.The receiver operating characteristic(ROC)curve was drawn to analyze the value of red blood cell parameters in the diagnosis of anemia.Results:The levels of RBC and Hb in the iron deficiency anemia group and Mediterranean anemia group were lower than those of the control group,and those of the iron deficiency anemia group were lower than those of the Mediterranean anemia group;the levels of RDW and MCV in the iron deficiency anemia group and Mediterranean anemia group were higher than the control group,and those of the iron deficiency anemia group were higher than those of the Mediterranean anemia group;and the differences were statistically significant(P<0.05).The ROC curve showed that the AUCs of RBC,RDW,Hb,MCV single and combined detection in the diagnosis of anemia were 0.840,0.766,0.816,0.690,and 0.938,separately.Among them,the AUCs of RBC,RDW,Hb single and combined detection were>0.7.The AUC of the combined detection was the highest;the AUC of MCV was less than 0.7,indicating poor efficacy in the diagnosis of anemia.Conclusions:Single and combined detection of RBC,RDW,and Hb have high efficacy in the diagnosis of anemia.The combined diagnosis has the highest efficiency.MCV has poor efficacy in the diagnosis of anemia.
