摘要
目的探究超前镇痛在骨折病患中的应用效果。方法选择濠江区新圣骨科医院于2020年9月-2021年12月收治的120例四肢长骨骨折病患,通过随机数字表分组法将患者分成S组(舒芬太尼组)30例,N组(纳布啡组)30例,SN组(舒芬太尼+纳布啡组)30例及C组(塞来昔布组)30例。统计四组的总体疗效、用药前、用药12h、24h、48h病患的疼痛评分(NRS)、镇静评分(OAA/S)、自控按压泵次数以及记录用药48h内追加曲马多镇痛的次数。结果四组药物的整体效果显著,且SN组的效果高于另外三组;服药后,各组的NRS、OAA/S评价均显著小于服药前,SN组用药后12h、24h、48h,NRS、OAA/S评价均显著小于S、N和C组;SN组术后1h、8h、12h、24h、48h时的吗啡使用率和术后追加镇痛频次、用量均显著小于另外三组(P<0.05)。结论对于骨折患者,超前镇痛可显著降低患者的疼痛,其中舒芬太尼+纳布啡在降低患者不良反应方面更明显,可显著减轻疼痛,促进疗效。
Objective To explore the effect of preemptive analgesia in fracture patients.Methods 120 patients with long bone fractures of extremities treated in Haojiang District Xinsheng orthopedic hospital from September 2020 to December 2021 were randomly divided into group S(sufentanil group),group N(naborphine group),group SN(sufentanil+naborphine group)and group C(celecoxib group).The overall efficacy of the four groups,the pain score(NRS),sedation score(OAA/S),the number of self-control compression pumps,and the number of additional tramadol analgesia within 48 hours of administration were recorded.Results The overall effect of four groups of drugs was significant,and the effect of SN group was higher than that of the other three groups.After administration,the NRS and OAA/S evaluations of each group were significantly lower than those before administration.At 12h,24h and 48h after administration,the NRS and OAA/S evaluations of SN group were significantly lower than those of S,N and C groups.The use rate of morphine and the frequency and dosage of postoperative additional analgesia in SN group at 1h,8h,12h,24h and 48h after operation were significantly lower than those in the other three groups(P<0.05).Conclusion For fracture patients,preemptive analgesia can significantly reduce the pain of patients,and sufentanil+naborphine is more obvious in reducing the adverse reactions of patients,which can significantly reduce the pain and promote the curative effect.
