摘要
目的探讨芎蛭通络胶囊治疗瘀血阻络型膜性肾病患者的临床效果及其对血清抗磷脂酶A2受体(PLA2R)抗体水平的影响。方法选取膜性肾病(瘀血阻络型)患者60例,将其随机分为对照组与观察组,每组30例。对照组给予常规治疗(醋酸甲泼尼龙片+环磷酰胺),观察组在常规治疗基础上给予芎蛭通络胶囊治疗,两组均治疗6个月。治疗6个月后,评估两组患者的临床疗效。比较两组患者治疗前后血清抗PLA2R抗体水平、24 h尿蛋白定量水平、血清白蛋白水平及凝血指标。结果治疗后,观察组总有效率高于对照组,血清抗PLA2R抗体水平、24 h尿蛋白定量水平、血小板计数均低于对照组,血清白蛋白水平高于对照组,凝血酶原时间、凝血酶时间均长于对照组(均P<0.05)。结论在常规治疗的基础上给予芎蛭通络胶囊治疗,可更有效地降低膜性肾病(瘀血阻络型)患者蛋白尿水平、血清抗PLA2R抗体水平,改善凝血功能,从而提高疗效。
Objective To explore the clinical effect of Xiongzhi Tongluo Capsule for the treatment of patients with membranous nephropathy in blood stasis and obstruction of collaterals type and its influence on serum anti phospholipase A2 receptor(PLA2R)antibody level.Methods A total of 60 patients with membranous nephropathy(type of blood stasis and obstruction of collaterals)were selected,and they were randomly divided into control group or observation group,with 30 cases in each group.The control group was given conventional treatment(methylprednisolone acetate+cyclophosphamide),based on which the observation group was treated with Xiongzhi Tongluo Capsule.Both groups received a six-month treatment.The clinical efficacy after 6 months of treatment was evaluated in both groups.Serum anti PLA2R antibody level,24-hour urine protein quantitation level,serum albumin level,and coagulation indexes before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the observation group was higher than that of the control group,and serum anti PLA2R antibody level,24-hour urine protein quantitation level,platelet counts in the observation group were lower than those in the control group,whereas serum albumin level was higher than that in the control group,as well as prothrombin time and thrombin time were longer than those in the control group(all P<0.05).Conclusion On the basis of conventional treatment,employing Xiongzhi Tongluo Capsule can effectively decrease proteinuria and serum anti PLA2R antibody levels in patients with membranous nephropathy(type of blood stasis and obstruction of collaterals),and ameliorate coagulation function,so as to improve efficacy.
